104例疑似犬异位性皮炎的回顾性研究

摘　　要：【目的】犬异位性皮炎是宠物临床常见的一种瘙痒性皮肤病,常因过敏、环境、遗传或免疫等因素引起。为确诊犬异位性皮炎,对临床104例疑似异位性皮炎患犬进行回顾性研究。【方法】采用四步排除法(跳蚤排除、其他外寄生虫排除、马拉色菌性皮炎和脓皮病排除、食物过敏排除)对来医院就诊的104例瘙痒患犬进行临床和实验室诊断。【结果】结果显示:62例患犬可确诊为异位性皮炎,其中小于3岁的、雄性、混血犬多发,临床多见轻度瘙痒,腹股沟、耳廓和腋下呈现红斑、苔藓化或脱毛;9例患犬对豚草、百喜草、石茅高粱等产生过敏阳性反应,4项观察指标与过敏原阳性病例数、以及过敏原特异性IgE总和之间无明显相关性。【结论】犬异位性皮炎需按照临床标准化流程的问诊、视诊、实验室检查和鉴别诊断等方法,结合四步排除法进行综合应用,可提高疾病确诊率,为临床对因治疗提供基础。Canine atopic dermatitis(CAD)in clinic is a common itching skin disease,caused by allergy,environmental,genetic or immune risk factors.The topic conducted a retrospective study on related cases in order to verify the characteristics of CAD.In this study,104 pruritus dogs in Meilianzhonghe Veterianry Hospital were comprehensively diagnosed by the clinical examination,and analyzed by four steps-exclusion method(flea exclusion,other parasites exclusion,Malassezia dermatitis and pyoderma exclusion,food allergy exclusion test).The results showed that 62 canine atopic dermatitis patients were diagnosed definitely,including less than 3 years old,male and mixed dogs.The clinical symptoms showed mild itching,erythema,moss or hair in groin,auricle and armpit.9 dogs had allergic positive reactions to ragweed,Bahia grass,citronella sorghum,and so on.There was no significant correlation between four observation indicators,the number of allergen positive cases,and allergen specific IgE quantitation.So,canine atopic dermatitis needs to be diagnosed according with clinical standardized process including inquiry,inspection,laboratory test and differential diagnosis,combined with the four-steps exclusion method.Using these mehods sequentially,we would make definital diagnosis and treat the cases targeting in pet clinic.
