摘 要:目的探讨血清脂联素(APN)、抗苗勒管激素(AMH)、性激素水平与儿童中枢性性早熟(CPP)的关系。方法选择宁波市妇女儿童医院2019年1月—2021年1月间收治的50例符合标准的CPP患者作为观察组研究对象,同期选择50例正常体检的儿童作为对照组对象;统计两组患者身高、体质量及骨龄等一般资料,检测受试者血中APN、AMH、雌二醇(E_(2))、卵泡刺激素(FSH)及黄体生成素(LH)水平;利用Spearman相关性检验分析APN、AMH、性激素与CPP的相关性,采用logistic回归模型分析APN、AMH、性激素在CPP发生的关系。结果观察组患者身高、体质量及骨龄均明显高于对照组,且差异有统计学意义(P<0.05);观察组患者血中APN明显低于对照组,AMH明显高于对照组,差异有统计学意义(P<0.05);观察组患者血中E_(2)、FSH及LH水平明显高于对照组,且差异有统计学意义(P<0.05);APN水平(r=0.634,P<0.05)与CPP呈明显负相关关系,AMH(r=0.586,P<0.05)、E_(2)(r=0.753,P<0.05)、FSH(r=0.601,P<0.05)及LH(r=0.645,P<0.05)与CPP呈显著正在相关关系,差异有统计学意义(P<0.05);血中低APN水平、高AMH、E_(2)、FSH及LH水平是影响CPP发生的独立性危险因素,差异有统计学意义(P<0.05)。结论APN水平与CPP呈明显负相关关系,AMH、E_(2)、FSH及LH与CPP呈显著正在相关关系,且血中低APN水平、高AMH、E_(2)、FSH及LH水平是影响CPP发生的独立性危险因素。Objective To select CPP patients admitted to our hospital as the research object to analyze the relationship between serum APN,AMH,sex hormone levels and CPP.Methods Fifty CPP patients who met the standard admitted in our hospital from January 2019 to January 2021 were selected as the observation group research objects,and 50 children with normal physical examination were selected as the control group during the same period;the height and weight of the two groups of patients were counted.Bone age and other general data,test subjects’blood levels of APN,AMH,E_(2),FSH and LH;use spearman correlation test to analyze the correlation between APN,AMH,sex hormones and CPP,and use logistic regression model to analyze APN,AMH,sex hormones The relationship that occurred in CPP.Results The height,body weight and bone age of the observation group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05);the blood APN of the observation group was significantly lower than that of the control group,and the AMH was significantly higher than that of the control group.There was statistical significance(P<0.05);the blood levels of E_(2),FSH and LH in the observation group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05);APN levels(r=-0.634,P<0.05)were significantly negatively correlated with CPP,AMH(r=0.586,P<0.05),E_(2)(r=0.753,P<0.05),FSH(r=0.601,P<0.05),LH(r=0.645,P<0.05)and CPP are significantly correlated,and the difference is statistically significant(P<0.05);low blood APN levels,high AMH,E_(2),FSH,and LH levels are independent threats that affect the occurrence of CPP,And the difference was statistically significant(P<0.05).Conclusion APN level is significantly negatively correlated with CPP,AMH,E_(2),FSH,and LH are significantly positively correlated with CPP,and low blood APN levels,high AMH,E_(2),FSH,and LH levels are independent threats that affect the occurrence of CPP.factor.
