摘 要:目的比较冷冻囊胚解冻移植与卵裂胚解冻培养囊胚移植对妊娠结局的影响。方法收集金华市人民医院生殖医学中心2017年1月—2018年12月冷冻囊胚解冻移植与卵裂胚解冻培养囊胚移植的病例,比较其移植率、妊娠率及流产率,之后又将卵裂胚解冻培养囊胚移植病例根据囊胚的评估结果分为4组,比较其移植率、妊娠率及流产率。结果冷冻囊胚解冻移植组的流产率(18.02%)明显高于卵裂胚解冻培养囊胚移植组(7.72%),而移植率和妊娠率没有明显差异,而冷冻囊胚解冻移植组的Ⅲ~Ⅳ期移植的妊娠率和流产率明显低于卵裂胚解冻培养囊胚移植组。卵裂胚解冻培养囊胚移植组组内比较显示,ⅢⅣ期移植的种植率、妊娠率都明显高于融合期与Ⅰ~Ⅱ期,Ⅴ~Ⅵ期移植的种植率明显高于融合期,妊娠率高于融合期与Ⅰ~Ⅱ期。结论在冻胚移植中,选择在冷冻后养囊或冷冻前养囊均可获得相似的移植率和妊娠率,但冷冻前养囊的流产率较高。Objective To compare the effect of vitrified-warmed blastocyst transfer and blastocyst derived from vitrified-warmed embryo transfer on pregnancy outcome.Methods The cases of vitrified-warmed blastocyst and blastocyst derived from vitrified-warmed embryo transfer from January 2017 to December 2018 in Jinhua People′s Hospital were collected.The transplantation rate,pregnancy rate and abortion rate were compared.According to the evaluation results of blastocyst derived from vitrified-warmed embryo transplantation cases was divided into 4 groups,the transplantation rate,pregnancy rate and abortion rate were compared.Results The abortion rate of vitrified-warmed blastocyst transfer group(18.02%)was significantly higher than blastocyst derived vitrified-warmed embryo transfer group(7.72%),and there was no significant difference in the transplantation rate and pregnancy rate.The pregnancy rate and abortion rate of stageⅢandⅣtransfer in blastocyst derived vitrified-warmed embryo transfer group were significantly lower than those in vitrified-warmed blastocyst transfer group.Comparison in vitrified-warmed blastocyst transfer group showed that the implantation rate and pregnancy rate of stageⅢandⅣtransfer were significantly higher than that of fusion stage,stageⅠandⅡ.The stageⅤandⅥtransfer had a higher rate of implantation compared to fusion stage,stageⅠandⅡ,and higher rate of pregnancy rate compared to fusion stage.Conclusion In frozen embryo transplantation,the choice of post-freezing or pre-freezing blastocyst can obtain similar transplantation rate and pregnancy rate,but the pre-freezing blastocyst have a higher abortion rate.
