摘 要:目的利用物联网便携式肺功能仪,对厦门市居民肺功能筛查,评估居民肺通气功能状况,探讨肺通气功能障碍的危险因素。方法利用物联网信息技术建立的呼吸云管理平台及赛客便携式肺功能检测仪,收集2018-2021年厦门市六个行政区各社区居民的基本信息并进行肺功能测定,评估居民肺通气功能状况,探讨肺通气功能障碍的危险因素。结果基本资料完整且肺功能数据合格的居民共61624人,有效率为69.94%。男女性别比为1:1.13,平均年龄50.25±14.53岁。肺通气指标第1秒呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC、最大呼气中期流速(MMEF)、呼气峰流速(PEF)平均值分别为(2.47±0.62)L/S、(3.20±0.76)L、(0.78±0.09)、(2.39±1.02)L/S、(5.25±1.57)L/S。FEV_(1)、FVC、MMEF、PEF男性高于女性,FEV_(1)/FVC男性低于女性。阻塞性通气功能障碍、限制性通气功能障碍、混合性通气功能障碍、小气道功能障碍检出率分别为13.84%、8.09%、1.54%、33.96%。男性、年龄是肺通气功能障碍的危险因素,BMI对不同类型的肺通气功能障碍影响不同。结论厦门市居民肺通气功能障碍比例较高,老年人尤为严峻,肺通气功能障碍与性别、年龄、BMI有关。基于物联网技术的大规模肺功能筛查有助于肺部通气功能障碍疾病的早期发现和预防。Objective To evaluate the pulmonary ventilation function of residents and explore the risk factors of pulmonary ventilation dysfunction by using internet-of-things based portable spirometerto screen the lung function of Xiamen residents.Methods The respiratory cloud management platform established by the internet-of-things and Xeekportable spirometer was used to collect the pulmonary ventilation function data and basic information of the residents of each community in the six administrative districts of Xiamen from 2018 to 2021,and to evaluate the pulmonary ventilation function of residentsand explore the risk factors of pulmonary ventilation dysfunction.Results A total of 61,624 residents had completed basic data and qualified pulmonary function data;with an effective rate of 69.94%.The sex ratio of male to female was 1∶1.13,and the average age was 50.25±14.53 years old.The pulmonary ventilation indicators including FEV_(1),FVC,FEV_(1)/FVC,MMEF,and PEF were(2.47±0.62)L/S,(3.20±0.76)L,(0.78±0.09),(2.39±1.02)L/S,(5.25±1.57)L/S,respectively.The FEV _(1),FVC,MMEF,and PEF of males were higher than those of females,and the FEV_(1)/FVC of males was lower than that of females.The detection rates of obstructive pulmonary ventilation dysfunction,restrictive ventilation dysfunction,mixed ventilation dysfunction,and small airway dysfunction were 13.84%,8.09%,1.54%,and 33.96%,respectively.Male and age are risk factors for pulmonary ventilation dysfunction,and BMI has different effects on different types of pulmonary ventilation dysfunction.Conclusion The proportion of pulmonary ventilation dysfunction in Xiamen residents is relatively high,especially in the elderly.Pulmonary ventilation dysfunction is related to gender,age,and BMI.Large-scale lung function screening based on IoT technology is helpful for the early detection and prevention of pulmonary ventilation dysfunction diseases.
