加味苍柏散联合甲钴胺注射液治疗糖尿病周围神经病变的临床研究

摘　　要：目的 观察加味苍柏散联合甲钴胺注射液治疗2型糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的临床疗效。方法 选取2020年6月—2021年12月在乌鲁木齐市中医医院就诊及住院的DPN患者80例,使用随机平行对照方法,将患者按随机数字表分为对照组和治疗组,各40例。对照组采用甲钴胺注射液,于双侧足三里穴位注射;治疗组在对照组基础上服用加味苍柏散。观测两组临床症状、HbA1c、Toronto临床评分系统、神经传导速度,包括运动神经传导速度(MNCV)、感觉神经传导速度(SNCV),不良反应。连续治疗4周后,判定疗效。结果 治疗组总有效率92.5%优于对照组总有效率82.5%,差异有统计学意义(P<0.05)。两组临床症状、糖化血红蛋白、Toronto临床评分系统、神经传导速度均有改善,且治疗组优于对照组,差异有统计学意义(P<0.05)。结论 加味苍柏散联合甲钴胺注射液治疗DPN效果显著,具有良好的安全性,值得临床推广。Objective To observe the clinical efficacy of Flavoured Cangbai Powder combined with Methylcobalamin injection in the treatment of type 2 diabetic peripheral neuropathy(DPN).Methods A total of 80 DPN patients who were treated and hospitalized in Urumqi Traditional Chinese Medicine Hospital from June 2020 to December 2021were selected.Using the random parallel control method,the patients were divided into the control group and the treatment group according to the random number table,with 40 cases in each group.The control group was injected with methylcobalamin injection at bilateral Zusanli acupoints.The treatment group took Flavoured Cangbai Powder on the basis of the control group.The clinical symptoms,HbA1c,Toronto clinical scoring system,nerve conduction velocity,including motor nerve conduction velocity(MNCV),sensory nerve conduction velocity(SNCV),and adverse reactions were observed in the two groups.After 4 weeks of continuous treatment,the curative effect was determined.Results The total effective rate of the treatment group was 92.5%,which was better than that of the control group,which was82.5%,and the difference was statistically significant(P<0.05).Clinical symptoms,glycosylated hemoglobin,Toronto clinical scoring system,and nerve conduction velocity were improved in both groups.And the treatment group was better than the control group,the difference was statistically significant(P<0.05).Conclusion Flavoured Cangbai Powder combined with Methylcobalamin injection in the treatment of DPN has a significant effect,has good safety,and is worthy of clinical promotion.
