视盘周围血流密度与糖尿病视网膜病变严重程度相关性的研究

摘　　要：目的 应用光学相干断层扫描血管成像技术(optical coherence tomography angiography,OCTA)分析不同阶段糖尿病视网膜病变(diabetic retinopathy,DR)患者视盘周围血流密度的变化。方法 选取2020年9月—2021年2月就诊蚌埠市第二人民医院的78例患者进行横断面研究。受试者根据早期治疗糖尿病视网膜病变研究(early treatment diabetic retinopathy study,ETDRS)标准分为4组:对照组(n=20)、无糖尿病视网膜病变组(n=21)、非增殖型糖尿病视网膜病变(n=19)和增殖型糖尿病视网膜病变(n=18)。应用OCTA对视盘周围及每个扇形区域的视网膜毛细血管血流密度(RVD)和脉络膜血管指数(CVI)进行测量和比较。结果 与对照组相比,无糖尿病视网膜病变组仅视盘颞侧RVD血流密度降低,差异有统计学意义(P<0.05),无糖尿病视网膜病变组CVI差异无统计学意义(P>0.05)。随着糖尿病视网膜病变的进展,各区域RVD和CVI均降低,差异有统计学意义(P<0.05)。相关性分析显示,视盘周围RVD和CVI与糖尿病视网膜病变严重程度相关(P<0.05)。结论 视盘周围的血流密度与糖尿病视网膜病变严重程度相关。Objective To analyze the changes of blood flow density around the optic disc in patients with different stages of diabetic retinopathy(DR) by optical coherence tomography angiography(OCTA).Methods A cross-sectional study of 78 patients who visited Bengbu Second People’s Hospital from September 2020 to February 2021 was selected.The subjects were divided into 4 groups according to the criteria of early treatment diabetic retinopathy study(ETDRS):control group(n=20),non-diabetic retinopathy group(n=21),non-proliferative diabetic retinopathy group Lesions(n=19) and proliferative diabetic retinopathy(n=18).Retinal capillary blood flow density(RVD) and choroidal vascular index(CVI) around the optic disc and in each sector were measured and compared using OCTA.Results Compared with the control group,in the non-diabetic retinopathy group,only the blood flow density of the optic disc temporal RVD was decreased,and the difference was statistically significant(P0.05).With the progression of diabetic retinopathy,RVD and CVI decreased in each region,and the difference was statistically significant(P<0.05).Correlation analysis showed that RVD and CVI around the optic disc were associated with the severity of diabetic retinopathy(P<0.05).Conclusion The blood flow density around the optic disc is related to the severity of diabetic retinopathy.
