摘　　要：【目的】探讨鹿瓜多肽注射液治疗膝关节骨性关节炎(KOA)患者的疗效。【方法】就诊于本院的110例KOA患者,随机分为观察组与对照组,每组55例。对照组采取常规疗法,观察组在对照组的基础上另给予鹿瓜多肽关节腔内注射治疗。比较两组治疗后的总有效率及治疗前后Lequesne评分、实验室相关指标[血沉(ESR)、C反应蛋白(CRP)、环氧酶-2(COX-2)、白介素-6(IL-6)]、滑膜液内白介素-18(IL-18)和基质金属蛋白酶-13(MMP-13)水平。【结果】观察组总有效率显著高于对照组(P<0.05);两组治疗后疼痛或不适、晨僵或起床后疼痛、步行时不适、最大步行距离、日常活动评分显著低于治疗前(P<0.05),且观察组治疗后上述评分均显著低于对照组(P<0.05);两组治疗后血清ESR、CRP、COX-2、IL-6水平显著低于治疗前(P<0.05),且观察组治疗后上述指标水平显著低于对照组(P<0.05);两组治疗后膜液内IL-18与MMP-13水平显著低于治疗前(P<0.05),且观察组显著低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。【结论】鹿瓜多肽注射液治疗KOA的疗效显著,可改善相关症状及实验室指标,降低滑膜液内IL-18与MMP-13水平,且不良反应发生率未明显增加。【Objective】To explore the efficacy of Lugua polypeptide injection in the treatment of knee osteoarthritis(KOA).【Methods】A total of 110 patients with KOA in our hospital were randomly divided into the observation group and the control group,with 55 cases in each group.The control group was treated with routine therapy,while the observation group was treated with intra-articular injection of Lugua polypeptide on the basis of the control group.The total effective rate,Lequesne score,laboratory related indexes(ESR,CRP,COX-2,IL-6),the level of interleukin-18(IL-18),and matrix metalloproteinase-13(MMP-13)in synovial fluid were compared between the two groups after treatment.【Results】The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The scores of pain or discomfort,morning stiffness or pain after getting up,discomfort during walking,maximum walking distance and daily activity score in the two groups after treatment were significantly lower than those before treatment(P<0.05),and the above scores in the observation group after treatment were significantly lower than those in the control group(P<0.05).The levels of serum ESR,CRP,COX-2 and IL-6 in the two groups after treatment were significantly lower than those before treatment(P<0.05),and the levels of the above indicators in the observation group after treatment were significantly lower than those in the control group(P<0.05).After treatment,the levels of IL-18 and MMP-13 in synovial fluid of the two groups were significantly lower than those before treatment(P<0.05),and their levels in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).【Conclusion】Lugua polypeptide injection has a significant effect in the treatment of KOA,which can improve relevant symptoms and laboratory indicators,reduce the levels of IL-18 and MMP-13 in synovial fluid.The incidenc
