内热针联合星状神经阻滞治疗颈源性头痛的临床研究

摘　　要：目的:探讨颈源性头痛患者采用内热针联合星状神经阻滞治疗的效果。方法:选取2020年1-12月湖南常德职业技术学院附属一医院收治的60例颈源性头痛患者作为研究对象,以随机数表法分为两组,各30例。对照组采用颈椎椎旁神经阻滞治疗,观察组采用星状神经阻滞联合内热针治疗。比较两组临床疗效、临床指标及并发症发生率。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组疼痛程度评分、关节活动度评分、头痛发作频率、搏动指数、阻力指数均低于对照组,头痛持续时间短于对照组,血流高速峰值高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:内热针联合星状神经阻滞治疗颈源性头痛的安全性、有效性较高,可使患者血流动力学指标明显改善,且临床症状及疼痛程度缓解,在临床中应用价值较高,值得推广。Objective:To investigate the treatment effect of internal heating acupuncture combined with stellate ganglion nerve block in patients with cervicogenic headache.Methods:A total of 60 patients with cervicogenic headache who were admitted to the First Affiliated Hospital of Changde Vocational Technical College from January 2020 to December 2020 were selected as the study subjects,and they were divided into two groups according to random digital number table method,with 30 cases in each group.The control group was treated with cervical paravertebral nerve block and the observation group was treated with stellate ganglion nerve block combined with internal heating acupuncture.The clinical efficacy,clinical indicators and complication rate of the two groups were compared.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The pain severity score,joint range of motion score,headache attack frequency,pulsation index and resistance index in the observation group were lower than those in the control group,the duration of headache was shorter than that in the control group,the peak blood flow velocity was higher than that in the control group,and the difference was statistically significant(P<0.05).The complication rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The combination of internal heating acupuncture with stellate ganglion nerve block in the treatment of cervicogenic headache has higher safety and effectiveness,it can significantly improve the hemodynamic index of patients,and alleviate the clinical symptoms and pain severity,and the application value in clinical practice is high,which is worth promoting.
