中药治疗输卵管炎性不孕症的用药规律及潜在作用机制

摘　　要：目的 基于数据挖掘分析中药治疗输卵管炎性不孕症的用药配伍规律,并通过网络药理学探讨常用药物的潜在作用机制。方法 在中国知网、万方和维普数据库检索治疗输卵管炎性不孕症的有效方剂。整理数据后进行药物频次分析,并对高频药物进行药效频次分析、性味归经频次分析、关联规则分析和系统聚类分析;使用BATMAN-TCM系统筛选出核心中药配伍所含化学成分及其潜在作用靶点,并对靶点进行疾病富集分析和信号通路分析。结果 (1)筛选出有效临床处方138首,包括药物168味。(2)高频药物(用药频次≥15次)共34味,其中当归、赤芍、丹参、三棱、路路通使用频次最高,常用中药类型为活血化瘀药、清热药、补虚药、行气药,药气以寒、温为主,药味以苦、辛居多,归经主归肝、心两经。(3)关联规则分析得到的17条药对组合,其中莪术→三棱为主要的药对;系统聚类分析将高频药物分为7类,涉及膈下逐瘀汤、桂枝茯苓丸、补阳还五汤等方剂。(4)在富集到的419种疾病中,炎症、疼痛与输卵管炎性不孕症的关系最大;在所富集的185条信号通路中,最有可能在治疗输卵管炎性不孕症中发挥作用的信号通路是磷酸肌醇-3-激酶/蛋白激酶B、叉头框O、瞬时受体电位信号通路。结论 中药治疗输卵管炎性不孕症以活血化瘀药为主,并随证配伍清热、行气、补虚等药物,莪术→三棱为核心中药配伍。核心药物配伍组合通过调控炎症反应和疼痛相关信号通路发挥治疗作用。Objective To analyze the rule of medication and combination of the herbal ingredients of Traditional Chinese Medicine in the treatment of salpingitis infertility based on data mining,and to explore the potential mechanism of common drugs through network pharmacology.Methods The effective prescriptions for treating salpingitis infertility were retrieved from Chinese National Knowledge Network(CNKI),Wanfang and Weipu(VIP)database.The medication frequency analysis was performed after collation of the data,and the frequency analysis of pharmaceutical effect,frequency analysis of property and flavour and meridian entry,association rules analysis,and systematic cluster analysis on high-frequency drugs were conducted;in addition,the Bioinformatics Analysis Tool of Molecular Mechanism of Traditional Chinese Medicine(BATMAN-TCM)system was used to screen chemical components belonged to the core combination of the herbal ingredients of Traditional Chinese Medicine and their potential effect targets,and the disease enrichment analysis and signaling pathway analysis were performed on the targets.Results(1)A total of 138 effective clinical prescriptions were screened out,including 168 drugs.(2)There were 34 high-frequency drugs(medication frequency≥15 times)in total,among which the medication frequency of Angelica sinensis,red peony root,Salvia miltiorrhiza,Sanleng and Liquidambaris fructus were the highest.The common types of Traditional Chinese Medicine contained drugs of promoting blood circulation to remove stagnation of blood,clearing heat,restoring qi,and moving qi.The property was mainly pathogenic cold and warm,the flavor was mainly bitter and acrid,and the meridian entries were mainly liver and heart meridians.(3)A total of 17 medicine pair combinations were obtained by the association rules analysis,thereinto Curcuma zedoary→Sanleng was the main medicine pair;furthermore,the high-frequency drugs were assigned to 7 types by the systematic cluster analysis,concerning prescriptions of Gexia Zhuyu Decoction,Guizhi Fu
