清浊化瘀方治疗原发性高血压合并冠心病痰瘀互结型的临床疗效及对患者血压、血脂的影响

摘　　要：目的 观察清浊化瘀方治疗原发性高血压合并冠心病痰瘀互结型的临床疗效及对患者血压、血脂的影响。方法 将120例原发性高血压合并冠心病痰瘀互结型患者按照随机数字表法分为2组,对照组60例予常规西药治疗,治疗组60例在对照组治疗基础上应用清浊化瘀方治疗,2组均治疗1个月。比较2组疗效;比较2组血压、血脂水平变化;比较2组治疗前后中医证候评分变化。结果 治疗组总有效率93.33%(56/60),对照组总有效率80.00%(48/60),治疗组疗效优于对照组(P<0.05)。2组治疗后收缩压(SBP)、舒张压(DBP)均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)均较本组治疗前降低(P<0.05),高密度脂蛋白胆固醇(HDL-C)较本组治疗前升高(P<0.05);治疗组治疗后TC、TG、LDL-C均低于对照组(P<0.05),HDL-C高于对照组(P<0.05)。2组治疗后胸痛、气短、心悸、眩晕、头痛评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论 在常规西药治疗基础上应用清浊化瘀方治疗原发性高血压合并冠心病痰瘀互结型,能改善患者血压、血脂及临床症状。Objective To observe the clinical efficacy of Qingzhuo Huayu Formula for essential hypertension(EH)complicated with coronary heart disease(CHD)of phlegm-blood stasis type and how it impacts on blood pressure(BP)and blood lipid.Methods Totally 120 patients with EH complicated with CHD(phlegm-blood stasis type)were randomly assigned into the treatment group(n=60)and the control group(n=60).All patients were managed by conventional Western medicine,and those in the treatment group were additionally treated with Qingzhuo Huayu Formula.The 1-month treatment was performed to compare BP,blood lipid,traditional Chinese medicine(TCM)syndrome score;the curative effect was assessed.Results The overall effective rate was higher in the treatment group(93.33%[56/60])compared with the control group(80.00%[48/60]),(P<0.05).After treatment,systolic blood pressure(SBP)and diastolic blood pressure(DBP)in groups were decreased,the treatment group was more prevalent(P<0.05);total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C)in groups were decreased(P<0.05),while high density lipoprotein cholesterol(HDL-C)in groups was increased(P<0.05);lower TC,TG and LDL-C levels,and higher HDL-C levels were detected in the treatment group than those in the control group(all P<0.05);TCM syndrome(chest pain,shortness of breath,palpitations,dizziness,headache)scores in groups were decreased(P<0.05),the treatment group was more common(P<0.05).Conclusion For patients with EH complicated with CHD(phlegm-blood stasis type),conventional Western medicine combined with Qingzhuo Huayu Formula can improve BP,blood lipids and clinical symptoms.
