中医综合治疗输卵管通而不畅导致不孕症的疗效观察

摘　　要：目的探讨中医综合治疗输卵管通而不畅导致不孕症的临床效果。方法150例输卵管通而不畅导致不孕症患者,经子宫输卵管四维超声造影(4D-HyCoSy)联合压力监测将通而不畅输卵管分3级:Ⅰ级48例,Ⅱ级52例,Ⅲ级50例;各分级患者按随机数字表法分为中医治疗组和西医治疗组:Ⅰ级患者每组24例,Ⅱ级患者每组26例,Ⅲ级患者每组25例。中医治疗组患者采用中医综合治疗,西医治疗组患者采用西医常规治疗。比较各级别中两组患者的不孕类型、临床效果、随访12个月妊娠情况,并比较各级别中中医治疗组患者的临床效果。结果各级别中两组的不孕类型比较差异均无统计学意义(P>0.05)。治疗后,Ⅰ级中医治疗组的总有效率为100.0%、西医治疗组的总有效率为83.3%;Ⅱ级中医治疗组的总有效率为92.3%、西医治疗组的总有效率为69.2%;Ⅲ级中医治疗组的总有效率为76.0%、西医治疗组的总有效率为48.0%。各级别内中医治疗组的总有效率均高于西医治疗组,差异具有统计学意义(P<0.05)。随访12个月后,Ⅰ级中医治疗组的宫内妊娠率为79.2%、西医治疗组的宫内妊娠率为50.0%;Ⅱ级中医治疗组的宫内妊娠率为61.5%、西医治疗组的宫内妊娠率为30.8%;Ⅲ级中医治疗组的宫内妊娠率为36.0%、西医治疗组的宫内妊娠率为12.0%。各级别内中医治疗组的宫内妊娠率均高于西医治疗组,差异具有统计学意义(P<0.05)。治疗3个月后,Ⅰ级、Ⅱ级、Ⅲ级中医治疗组的总有效率分别为100.0%、92.3%、76.0%;Ⅰ级、Ⅱ级中医治疗组的总有效率均高于Ⅲ级中医治疗组,差异具有统计学意义(P<0.05)。结论输卵管通而不畅超声造影分级为中医制定合理的临床诊疗方案提供了更清晰的思路,使中医综合治疗输卵管通而不畅导致不孕的疗效更显著,值得临床推广应用。Objective To discuss the clinical effect of traditional Chinese medicine on comprehensive treatment of infertility caused by incomplete patency oviduct.Methods 150 patients with infertility due to tubal incompetence were classified into 3 grades by four-dimensional hysterosalpingo-contrast sonograpy(4D-HyCoSy)combined with pressure monitoring:48 cases of gradeⅠ,52 cases of gradeⅡ,and 50 cases of gradeⅢ.Patients of each grade were divided into Chinese medicine treatment group and western medicine treatment group according to the random numerical table:24 cases in each group of gradeⅠpatients,26 cases in each group of gradeⅡpatients,and 25 cases in each group of gradeⅢpatients.Patients in the traditional Chinese medicine treatment group were treated with comprehensive traditional Chinese medicine,and patients in the western medicine treatment group were treated with conventional western medicine.The types of infertility,clinical effect,and 12-month follow-up pregnancy were compared between the two groups at each grade,and the clinical effects of patients in the traditional Chinese medicine treatment group at each grade were compared.Results There was no statistically significant difference in the types of infertility between the two groups in all grades(P>0.05).After treatment,in gradeⅠpatients,the total effective rate of the traditional Chinese medicine treatment group was 100.0%,and the total effective rate of the western medicine treatment group was 83.3%;in gradeⅡpatients,the total effective rate of the traditional Chinese medicine treatment group was 92.3%,and the total effective rate of the western medicine treatment group was 69.2%;in gradeⅢpatients,the total effective rate of the traditional Chinese medicine treatment group was 76.0%,and the total effective rate of the western medicine treatment group was 48.0%.The total effective rate of the traditional Chinese medicine treatment group was higher than that of the western medicine treatment group in all grades,and the differences were stat
