摘 要:目的分析降尿酸药治疗慢性肾脏病合并高尿酸血症的临床效果。方法50例慢性肾脏病合并高尿酸血症患者,随机分为对照组和观察组,各25例。对照组给予别嘌醇治疗,观察组给予非布司他治疗。比较两组尿酸恢复正常时间、蛋白尿消失时间、治疗效果、不良反应发生率及治疗前后血尿酸、血肌酐、尿素氮水平。结果观察组尿酸恢复正常时间(5.25±1.31)d、蛋白尿消失时间(8.34±0.12)d均短于对照组的(7.25±2.61)、(10.54±1.35)d,差异有统计学意义(P<0.05)。治疗前,两组血肌酐、尿素氮、血尿酸水平比较,差异无统计学意义(P>0.05);治疗后,观察组血肌酐(148.06±13.24)μmol/L、尿素氮(6.15±1.35)mmol/L、血尿酸(254.42±10.12)μmol/L均低于对照组的(165.34±23.91)μmol/L、(7.21±1.45)mmol/L、(285.55±13.21)μmol/L,差异有统计学意义(P<0.05)。观察组治疗总有效率96.00%高于对照组的72.00%,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论相对于别嘌醇,采取非布司他治疗慢性肾脏病合并高尿酸血症的效果更确切,可有效改善肾功能,降低血尿酸水平,且不会增加不良反应,值得推广和应用。Objective To analyze the clinical effect of uric acid-lowering drugs in the treatment of chronic kidney disease with hyperuricemia.Methods A total of 50 patients with chronic kidney disease complicated with hyperuricemia were randomly divided into the control group and the observation group,with 25 cases in each group.Patients in the control group were treated with allopurinol,and patients in the observation group were treated with febuxostat.Both groups were compared in terms of recovery time of uric acid,disappearance time of proteinuria,therapeutic effect,incidence of adverse reactions,and the levels of serum uric acid,serum creatinine,and blood urea nitrogen before and after treatment.Results The recovery time of uric acid(5.25±1.31)d and the disappearance time of proteinuria(8.34±0.12)d in the observation group were shorter than(7.25±2.61)and(10.54±1.35)d in the control group,and the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in the levels of serum creatinine,blood urea nitrogen and serum uric acid between the two groups(P>0.05).After treatment,the serum creatinine(148.06±13.24)μmol/L,blood urea nitrogen(6.15±1.35)mmol/L,blood uric acid(254.42±10.12)μmol/L in observation group were lower than(165.34±23.91)μmol/L,(7.21±1.45)mmol/L,(285.55±13.21)μmol/L in control group,and the differences were statistically significant(P<0.05).The total effective rate 96.00%of the observation group was higher than 72.00%of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Compared with allopurinol,febuxostat is effective in the treatment of chronic kidney disease with hyperuricemia,which can effectively improve renal function and reduce blood uric acid level without increasing adverse reactions.It is worthy of promotion and application.
