清热泄浊方联合苯溴马隆治疗无症状高尿酸血症湿热证疗效及对血清炎性因子的影响

摘　　要：目的观察清热泄浊方联合苯溴马隆治疗无症状高尿酸血症(Hyperuricemia,HUA)湿热证疗效及对血清炎性因子的影响。方法选取2018年4月—2019年4月期间北京中医药大学东方医院收治的无症状HUA患者82例,分为对照组40例和治疗组42例,对照组脱落病例1例。两组患者均给予碳酸氢钠治疗,对照组加用苯溴马隆治疗,治疗组在对照组基础上加用清热泄浊方治疗。治疗4周后,观察比较两组患者临床疗效、不良反应,治疗前后中医证候积分、血尿酸水平、血清炎性因子[肿瘤坏死因子α(Tumor necrosis factorα,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-8(Interleukin-8,IL-8)]水平。结果治疗后治疗组总有效率85.71%(36/42)明显高于对照组66.67%(26/39),差异有统计学意义(P<0.05)。治疗后两组患者中医证候积分均较治疗前降低,差异有统计学意义(P<0.05);且治疗组中医证候积分明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血尿酸水平均较治疗前降低,差异有统计学意义(P<0.05);且治疗组血尿酸水平明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清IL-6、IL-8及TNF-α水平均较治疗前降低,差异有统计学意义(P<0.05);且治疗组血清IL-6、IL-8及TNF-α水平均明显低于对照组,差异有统计学意义(P<0.05)。两组患者治疗过程中均未出现药物相关不良反应。结论清热泄浊方联合碳酸氢钠治疗无症状HUA的疗效显著,能有效改善临床症状,降低血尿酸及血清炎性因子水平,且安全性可靠,值得临床推广使用。Objective To observe the efficacy of Qingre Xiezhuo Formula combined with sodium bicarbonate in the treatment of asymptomatic hyperuricemia(HUA)with the damp-heat syndrome and the influence on serum inflammatory factors.Methods Eighty-two asymptomatic patients with HUA admitted to Dongfang Hospital,Beijing University of Chinese Medicine from April 2018 to April 2019 were included.They were divided into a control group(40 cases)and a treatment group(42 cases),and 1 case fell off in the control group.Patients in both groups were treated with sodium bicarbonate,and patients in the control group were additionally given benzbromarone.Based on the control group,patients in the treatment group were treated with Qingre Xiezhuo Formula.After 4 weeks of treatment,the clinical efficacy and adverse reactions of the two groups were observed and compared.The scores of TCM syndromes and the levels of blood uric acid and inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-8(IL-8)]in the serum of patients in the two groups were compared before and after treatment.Results The total effective rate of the treatment group[85.71%(36/42)]was higher than that of the control group[66.67%(26/39)](P<0.05).After treatment,the scores of TCM syndromes in both groups were lower than those before treatment(P<0.05),and the score of TCM syndromes in the treatment group was lower than that in the control group(P<0.05).After treatment,the levels of blood uric acid in both groups were lower than those before treatment(P<0.05),and the treatment group was lower than the control group(P<0.05).After treatment,the levels of IL-6,IL-8,and TNF-αin the serum of patients in both groups were lower than those before treatment(P<0.05),and the treatment group was lower than the control group(P<0.05).No adverse reaction occurred in the two groups during the treatment course.Conclusion Qingre Xiezhuo Formula combined with sodium bicarbonate is effective in the treatment of asymptomatic HUA,which can effectively improve the cl
