摘 要:目的观察仿真生物电穴位刺激联合虚拟情景康复训练对脑卒中患者神经功能、脑细胞代谢情况的影响。方法选取2018年1月—2019年12月期间南通大学第二附属医院收治的240例脑卒中患者为研究对象,按随机数字表法分为电针组与联合组,每组各120例。电针组接受仿真生物电穴位刺激,联合组在此基础上联合虚拟情景康复训练。治疗3周后,观察比较两组患者治疗前后的神经功能[美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分]、日常生活活动能力[改良Barthel指数(Modified Barthel Index,MBI)]、上肢运动功能[Fugl-Meyer运动功能(Fugl-Meyer motor function assessment,FMA)评定量表评分]、平衡功能[Berg平衡量表(Berg balance scale,BBS)]、步行能力[功能性步行量表(Functional ambulation category scale,FAC)]、生活质量[生活质量测定量表(WHO Quality of Life-100,WHOQOL-100)]及血清超氧化物歧化酶(Superoxide dismutase,SOD)、丙二醛(Malonaldehyde,MDA)、一氧化氮(Nitric oxide,NO)、氧化氮合成酶(Nitric oxide synthase,NOS)、脑源性神经营养因子(Brain-derived neurotrophic factor,BDNF)、神经生长因子(Nerve growth factor,NGF)、神经营养素-3(Nerve nutrients-3,NT-3)水平。结果治疗后两组患者NIHSS评分均明显低于治疗前,Barthel指数均明显高于治疗前,差异有统计学意义(P<0.05);且联合组NIHSS评分明显低于电针组,Barthel指数明显高于电针组,差异有统计学意义(P<0.05)。治疗后两组患者上肢FMA评分均明显高于治疗前,差异有统计学意义(P<0.05);且联合组上肢FMA评分明显高于电针组,差异有统计学意义(P<0.05)。治疗后两组患者BBS、FAC评分均明显高于治疗前,差异有统计学意义(P<0.05);且联合组明显高于电针组,差异有统计学意义(P<0.05)。治疗后两组患者血清SOD、NO、NOS水平均明显高于治疗前,MDA水平均明显低于治疗前,差异有统计学意义(P<0.05);且�Objective To explore the effects of simulated bioelectric stimulation combined with virtual rehabilitation training on nerve function and brain cell metabolism in stroke patients.Methods A total of 240 stroke patients admitted to The Second Affiliated Hospital of Nantong University/Nantong First People′s Hospital from January 2018 to December 2019 were selected as the study subjects,and they were divided into the electric needle group and the combination group by random number table,120 in each group.The electric needle group received simulated bioelectrical stimulation,and the combination group was given combined virtual rehabilitation training on this basis.Patients in both groups were continuously intervened for 3 weeks.After 3 weeks,the following items of the two groups were analyzed and compared:nerve function[National Institutes of Health stroke scale(NIHSS)],activities of daily life[Modified Barthel Index(MBI)],upper limb motor[Fugl-Meyer Assessment(FMA)],balance function[Berg balance scale(BBS)],walking ability[Functional Ambulation Category(FAC)],quality of life[WHO Quality of Life-100(WHOQOL-100)],serum superoxide dismutase(SOD),malondialdehyde(MDA),nitric oxide(NO),nitric oxide synthase(NOS),brain derived neurotrophic factor(BDNF),and nerve nutrients-3(NT-3).Results After intervention,the NIHSS score of the combination group was lower,and MBI,FMA score,BBS score and FAC score were higher than those of the electric needle group(all P<0.05),and the combination group had more decrease or increase than the control group(P<0.05);serum SOD,NO and NOS,BDNF,NGF,and NT-3 levels of the combination group were higher while the MDA level was lower than those of electric needle group(all P<0.05),and the combination group had more elevation or reduction than the electric needle group(P<0.05);the two groups had increased scores in health status,cognitive function,physiological function and overall health compared with the conditions before treatment(P<0.05);all dimensions of WHOQOL-100 in the combination group were
