摘 要:目的 探讨灼手综合征手术的疗效及预后相关因素。方法 手术治疗32例外伤性灼手综合征患者。采用GRASSP评分系统(背侧感觉、掌侧感觉、肌力、抓握能力、抓握性能)评估上肢神经功能情况,采用Pearson积矩相关系数对神经功能恢复相关因素进行相关性分析。结果 患者均获得随访,时间2~7年。术后1周及末次随访时上肢背侧感觉、掌侧感觉、肌力、抓握能力、抓握性能评分情况均较术前显著改善(P<0.05)。末次随访时上肢肌力和抓握性能的恢复与术前相应神经功能评分呈正相关(P<0.05)。结论 外伤性灼手综合征患者通过减压内固定手术可获得较好的神经功能恢复,上肢肌力和抓握性能的恢复情况与术前相应神经功能评分呈正相关。Objective To investigate the related factors of surgery effect and prognostic related factors of burning hand syndrome.Methods Surgical treatment was performed for 32 patients with traumatic burning hand syndrome. The GRASSP score system(dorsal sensation,volar sensation,muscle strength,grasping ability,grasping performance) was used to evaluate the neurological function of the upper limb,and Pearson product moment correlation coefficient was used to calculate the correlation of neurological function recovery.Results All patients were followed up for 2 ~ 7years. The scores of dorsal sensation,volar sensation,muscle strength,grasping ability and grasping performance of upper limbs were significantly improved at 1 week after operation and the last follow-up,compared with the preoperation(P< 0. 05). At the last follow-up,the recovery of upper limb muscle strength and grasping performance were positively correlated with preoperative corresponding nerve functional scores(P< 0. 05).Conclusions Patients with traumatic burning hand syndrome can obtain better recovery of neurological function through decompression and internal fixation,and the recovery of upper limb muscle strength and grasping performance are positively correlated with preoperative corresponding neurological function scores.
