摘 要:目的 探讨浸骨水泥明胶海绵填充在经皮椎体成形术(PVP)治疗老年胸腰椎骨折中的应用效果。方法PVP中应用浸骨水泥明胶海绵填充治疗21例老年胸腰椎骨折患者。记录手术时间、骨水泥注入量、术中骨水泥渗漏及并发症发生情况。采用疼痛VAS评分、伤椎Cobb角、伤椎中部高度百分比和伤椎前缘高度百分比评估手术疗效。采用改良MacNab评分评估手术优良率。结果 患者均获得随访,时间3~36个月。术后未发生神经损伤、肺栓塞、骨水泥反应等并发症。疼痛VAS评分、伤椎Cobb角、伤椎中部高度百分比、伤椎前缘高度百分比术后均明显优于术前(P<0.05)。末次随访时采用改良MacNab评分评估手术优良率为21/21。结论 PVP中应用浸骨水泥明胶海绵填充治疗老年胸腰椎骨折,不仅椎体后凸畸形改善、椎体高度恢复满意,而且骨水泥渗漏率低。Objective To investigate the application effect of bone cement impregnated gelatin sponge filling in percutaneous vertebroplasty(PVP) for thoracolumbar fractures in the elderly.Methods The 21 elderly patients with thoracolumbar fractures were treated with bone cement impregnated gelatin sponge filling in PVP. The time of operation,injection volume of bone cement, intraoperative bone cement leakage and complications were recorded. Pain VAS,Cobb angle of the injured vertebrae,the percentage of midline height and percentage of anterior edge height of the injured vertebrae were used to evaluate the surgical outcome. Evaluation of surgical results was performed by modified MacNab score.Results All patients were followed up for 3 ~ 36 months. No complications such as nerve injury,pulmonary embolism and bone cement reaction occurred. Pain VAS,Cobb angle, percentage of midline height, and percentage of anterior edge height of the injured vertebrae were significantly better than those before operation(P<0. 05). At the last follow-up,the excellent-good rate was 21/21 according to modified MacNab score.Conclusions In the treatment of elderly thoracolumbar fractures,bone cement impregnated gelatin sponge filling are used in PVP,not only kyphosis deformity improvement and vertebral height are restored satisfactorily, but also the leakage rate of bone cement is low.
