健脾化痰汤治疗慢性阻塞性肺疾病稳定期肺脾两虚证的效果及对HIF-1α、NLRP3水平的影响

摘　　要：目的 探讨健脾化痰汤治疗慢性阻塞性肺疾病稳定期肺脾两虚证的效果。方法 选取2018年1月至2019年12月我院收治的80例慢性阻塞性肺疾病稳定期肺脾两虚证患者作为研究对象,根据治疗方案不同将其分为对照组和观察组,各40例。对照组给予常规西医治疗,观察组在对照组基础上加用健脾化痰汤。比较两组的治疗效果。结果 治疗后,观察组的第1秒用力呼气容积占预计值百分比(FEV%pred)、用力肺活量占预计值百分比(FVC%pred)、第1秒用力呼气容积与用力肺活量的比值(FEV/FVC)高于对照组(P<0.05)。治疗后,观察组的基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶-2(MMP-2)水平低于对照组(P<0.05)。治疗后,观察组的缺氧诱导因子1α(HIF-1α)、核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)水平低于对照组(P<0.05)。结论 健脾化痰汤能调节慢性阻塞性肺疾病稳定期肺脾两虚证患者的MMP-9、MMP-2、HIF-1α及NLRP3水平,促进其肺功能恢复。Objective To explore the effect of Jianpi Huatan decoction in the treatment of chronic obstructive pulmonary disease in stable phase with lung-spleen deficiency. Methods A total of 80 patients with chronic obstructive pulmonary disease in stable phase with lung-spleen deficiency admitted in our hospital from January 2018 to December 2019 were selected as the research objects, and the patients were divided into control group and observation group according to different treatment schemes, with 40 cases in each group. The control group was given conventional western medicine treatment, and the observation group was treated with Jianpi Huatan decoction on the basis of the control group. The therapeutic effects of the two groups were compared. Results After treatment, forced expiratory volume in one second as percentage of predicted value(FEV%pred), forced vital capacity as a percentage of predicted value(FVC%pred) and ratio of forced expiratory volume in one second to forced vital capacity(FEV/FVC) in the observation group were higher than those in the control group(P<0.05). After treatment, the levels of matrix metalloproteinase-9(MMP-9) and matrix metalloproteinase-2(MMP-2) in the observation group were lower than those in the control group(P<0.05). After treatment,the levels of hypoxia induce factor 1α(HIF-1α) and NOD-like receptor protein 3(NLRP3) in the observation group were lower than those in the control group(P<0.05). Conclusion Jianpi Huatan decoction can regulate the MMP-9, MMP-2,HIF-1α and NLRP3 levels of patients with chronic obstructive pulmonary disease in stable phase with lung-spleen deficiency, and promote recovery of lung function.
