熵权TOPSIS联合RSR法评价某三级医院抗菌药物管理质量

摘　　要：目的:评价医院抗菌药物管理质量,为抗菌药物管理提供数据参考。方法:从江苏省某三级医院药学部及感控科获取2012年-2021年抗菌药物相关的4个维度10个指标,利用熵权TOPSIS法的C值及RSR法的RSR值联合评价抗菌药物管理水平,并通过两种方法的排序结果分析10年管理水平走势。结果:2020及2021年该院抗菌药物管理处于RSR模型的优势档(3档),C值分别为0.912和0.938;2015年处于劣档(1档),C值为0.237;其余年份处于中档(2档),数据呈两头小中间大的枣核型;10年总体管理水平呈3个阶段曲折上升,第一阶段2012年-2015年,C值缓慢上升,但均低于0.3;第二阶段为2016年-2018年,C值分别是0.425、0.298、0.321,处于波动状态;第三阶段为2019年-2021年,C值稳步上升,并达到了较高水平,2020及2021年达到了0.9以上;相关性分析显示两种模型结果存在强相关(r=0.942,P<0.05)。结论:该院抗菌药物管理水平总体呈走势良好,熵权TOPSIS联合RSR评价抗菌药物管理水平结果科学可信,可作为决策层管理依据。Objective:To evaluate the quality of antimicrobial management in a hospital,and to provide data reference for antimicro⁃bial drugs management.Methods:10 indicators in 4 dimensions related to antibiotics from 2012 to 2021 were obtained from the depart⁃ment of Pharmacy and department of infection controlin a certain tertiary hospital in Jiangsu,and the C value of the entropy TOPSIS method and the RSR value of the RSR method were used to jointly evaluate the management level of antibiotics.The ranking results of the method analyze the trend of management level in 10 years.Results:In 2020 and 2021,the antibacterial drug management depart⁃ment of our hospital was in the superior grade(3 grades)of the RSR model,and the C values were 0.912 and 0.938,respectively.In 2015,it was in the inferior grade(1 grade),and the C value was 0.237.The rest of the years are in the mid-range(level 2),the data is a jujube karyotype with two small ends and a large middle.The overall management level in 10 years has tortuousrisen in three stages.In the first stage,from 2012 to 2015,the C value rose slowly,but all were lower than 0.3;The stage is 2016-2018,the C values are 0.425,0.298,and 0.321,respectively,and the C value is in a state of fluctuation;the third stage is 2019-2021,and the C value has steadily increased and reached a higher level.In 20 and 21 reached above 0.9.Correlation analysis showed that there was a strong cor⁃relation between the results of the two models(r=0.942,P<0.05).Conclusion:The overall trend of antimicrobial drugsmanagement in our hospital is good.The results of entropy-weight TOPSIS combined with RSR to evaluate antimicrobialdrugsmanagement are scientif⁃ic and credible,and are worthy of decision-making as the management basis.
