冰红消肿膏联合冲击波治疗冻结肩的疗效观察

摘　　要：目的探讨冰红消肿膏联合冲击波对冻结肩患者疼痛和功能的影响,为治疗冻结肩提供临床支持。方法选取2016年3月—2019年6月期间北京市大兴区中西医结合医院骨伤科收治的冻结肩恢复期患者70例,采用随机数字表法分为治疗组和对照组,每组各35例。对照组进行冲击波和功能锻炼,治疗组在对照组基础上联合冰红消肿膏治疗。治疗5周后,观察比较治疗前后两组患者收集肌力、疼痛(VAS)、日常生活能力、关节活动度、肩关节评分量表(Constant-Murley,CMS)、肌骨超声肩胛下肌肌腱(Subscapularis tendon,SubS)、三角肌肌腱(Deltoid tendon,Dt)、冈上肌肌腱(Supraspinatus tendon,SupraS)指标的改善情况。结果治疗后治疗组总有效率91.43%(32/35)较对照组80.00%(28/35)明显升高,差异有统计学意义(P<0.05)。两组患者治疗后VAS评分均较治疗前降低,肌力评分、日常生活评分均较治疗前升高,差异有统计学意义(P<0.05);且治疗组VAS评分较对照组明显降低,肌力评分、日常生活评分均较对照组明显升高,差异有统计学意义(P<0.05)。两组患者治疗后关节活动度评分均较治疗前明显升高,差异有统计学意义(P<0.05);且治疗组较对照组明显升高,差异有统计学意义(P<0.05)。两组患者治疗后肌腱厚度SubS、Dt、SupraS指标均较治疗前明显降低,差异有统计学意义(P<0.05);且治疗组较对照组明显降低,差异有统计学意义(P<0.05)。结论冰红消肿膏联合冲击波治疗冻结肩,冲击波治疗后,对肩关节粘连松解明显,采用冰红消肿膏消肿、镇痛,可有效改善肩关节功能。Objective To explore the effect of Binghong Xiaozhong Ointment combined with shockwave therapy on pain and function of frozen shoulder patients,and thus to provide a reference for the treatment of frozen shoulder.Methods A total of 70 patients with frozen shoulder at the recovery stage who were treated in the Department of Orthopedics and Traumatology,Beijing Daxing District Hospital of Integrated Chinese and Western Medicine from March 2016 to June 2019 were randomized into the observation group(n=35)and control group(n=35).The control groupreceivedshockwave therapy and performed functional exercise.In addition to theshockwave therapy and functional exercise in the control group,Binghong Xiaozhong Ointmentwas used in the observation group.After 5 weeks of treatment,themuscle strength,Visual Analogue Scale(VAS)score,activities of daily living(ADLs),range of motion(ROM),Constant-Murley score(CMS),subscapularis tendon(Subs),deltoid tendon(Dt),and supraspinatus tendon(Supras)before and after treatment were compared between the two groups.Results After treatment,the total effective rate was 91.43%(32/35)in the observation group,as compared with the 80.00%(28/35)in the control group(P<0.05).The VAS score was lower and the muscle strength score and ADLs score were higher than those before treatment in both groups(P<0.05),particularly in the observation group which showed significant difference from the control group(P<0.05).After treatment,the score of joint motion was significantly higher than that before treatment in both groups(P<0.05),and the score in the observationgroup was higher than that in the control group(P<0.05).Subs,CAL,and Supras were decreased in both groups after treatment compared with those before treatment(P<0.05),and they were lower in the observation group than in the control group(P<0.05).Conclusion The combination of Binghong Xiaozhong Ointment and shockwave therapy is effective on frozen shoulder,as Binghong Xiaozhong Ointment can eliminate swelling and ease pain and shockwave therapy relieves
