小针刀联合委中穴刺络放血治疗风寒湿阻型腰背肌筋膜炎的临床疗效观察

摘　　要：目的:观察小针刀联合委中穴刺络放血治疗风寒湿阻型腰背肌筋膜炎的临床疗效。方法:将60例腰背肌筋膜炎患者随机分为观察组和对照组,每组30例。对照组行常规针刺联合委中穴刺络放血治疗,观察组行小针刀联合委中穴刺络放血治疗,疗程均为3周。分别于治疗前、治疗1周、治疗结束,采用简化McGill疼痛问卷(SF-MPQ)评价疼痛程度,中医证候积分评定中医证候疗效,并在治疗结束后评估临床疗效。结果:治疗结束,观察组临床疗效显著优于对照组(P<0.05);治疗1周、治疗结束及随访3个月,两组患者中医证候积分及McGill疼痛问卷(SF-MPQ)的PRI、VAS、PPI评分和总分均较本组治疗前显著降低(P<0.05);观察组患者中医证候积分及McGill疼痛问卷(SF-MPQ)的PRI、VAS、PPI评分和总分均显著低于对照组(P<0.05)。结论:小针刀联合委中穴刺络放血治疗风寒湿阻型腰背肌筋膜炎能有效降低患者中医证候积分及改善疼痛程度,且临床疗效优于针灸联合委中穴刺络放血。Objective:To observe the clinical effect of small-needle-knife combined with Weizhong point(BL40)pricking and bloodletting for lumbar back myofascitis with wind-cold-dampness obstruction pattern.Methods:Sixty patients with lumbar back myofascitis were randomly divided into an observation group and a control group,with 30 cases in each group.The control group was treated with conventional acupuncture combined with Weizhong point(BL40)pricking and bloodletting,and the observation group was treated with small-needle-knife combined with Weizhong point(BL40)pricking and bloodletting.The duration of the treatment was 3 weeks in all cases.Before treatment,after one week of treatment,and at the end of treatment,the simplified McGill pain questionnaire(SF-MPQ)was used to evaluate the degree of pain,and the TCM syndrome score was used to evaluate the efficacy of TCM syndromes,and the clinical efficacy was assessed after the treatment.Results:After the treatment,the clinical efficacy of the observation group was significantly better than that of the control group(P<0.05);after one week of treatment,at the end of treatment and at the 3-month follow-up,the TCM syndrome score and the PRI,VAS,PPI scores and total score of SF-MPQ were significantly lower in both groups compared with those before treatment(P<0.05);the TCM syndrome score and the PRI,VAS,PPI scores and total score of SF-MPQ in observation group were significantly lower than those in control group(P<0.05).Conclusion:Small-needle-knife combined with Weizhong point(BL40)pricking and bloodletting for lumbar back myofascitis with wind-cold-dampness obstruction pattern can effectively reduce the TCM syndrome score and improve the pain degree,and the clinical effect is better than that of acupuncture combined with Weizhong point pricking and bloodletting.
