基于“火郁发之”理论探讨针灸外治法在急性期带状疱疹中的应用

摘　　要：带状疱疹急性期湿热毒邪郁于肌表,若医者用药多以苦寒直折,往往收效不甚理想。而基于“火郁发之”理论,使用火针、铺棉灸、艾灸、叩刺拔罐等温热刺激外治法,可同气相求,引内郁之热毒透散,温通行气、活血化瘀、破坚消积、祛火之源、扶正祛邪,收效甚好。针对针灸外治法治疗急性带状疱疹的机理进行深入探讨,并结合现代研究成果,为“火郁发之”的理论探寻科学基础。温度敏感型瞬时受体电位(transient receptor potentials,TRP)通道及机械敏感性离子通道均参与了火针发挥疗效的过程,并起到重要的作用,有待进一步研究印证。In the acute stage of herpes zoster,the dampness-heat-toxin are depressed in the body surface.If the doctors use herbal medicines with bitter and cold properties,the treatment effect is often not satisfactory.Based on the theory of“dispersing the stagnation of fire”,the use of fire-needling,cotton-moxibustion,moxibustion,puncture-cupping and other warm-heat stimulative external treatment methods could out-thrust and disperse heat-toxin,warm and dredge the movement of qi,activate blood and resolve stasis,break down the solid stagnation,dispell the source of fire,strengthen the healthy qi and eliminate the pathogenic.The effect is excenent.In this paper,we discuss these external acupuncture treatment methods for acute herpes zoster in depth and combine modern research results to explore the scientific basis for the theory of“dispersing the stagnation of fire”.The temperature-sensitive transient receptor potentials(TRP)channel and the mechanosensitive ion channel are involved and play an important role in the healing process of five-needling,which needs to be corroborated by further studies.
