武汉地区2型糖尿病患者葡萄糖负荷后血糖、胰岛素及C肽水平差异性分析

摘　　要：目的探讨武汉地区2型糖尿病(T2DM)患者葡萄糖负荷后,血清中血糖(PG)、胰岛素(INS)及C肽(CP)水平的差异性。方法选取2020年11月至2021年10月湖北省中医院(以下简称“我院”)收治的191例T2DM患者作为研究组,另选取同期我院收治的非糖尿病患者74例作为对照组。两组均空腹8~10 h后,5 min内口服75 g葡萄糖,收集并比较所有研究对象空腹及葡萄糖负荷后0.5、1、2、3 h时的PG、INS、CP水平。根据T2DM不同疾病进展将研究组患者分为T2DM无并发症组(144例)和T2DM伴并发症组(47例),同时将对照组中21例诊断为糖尿病前期的患者作为糖尿病前期组,比较三组间各检测指标是否存在差异性。另根据不同并发症将T2DM伴并发症组的患者分为周围神经病变组(11例)、视网膜病变组(9例)、酮症酸中毒组(8例)、大血管病变组(9例)、糖尿病性肾病组(10例),比较五组间各检测指标是否存在差异性。结果研究组空腹PG、0.5hPG、1hPG、2hPG、3hPG均高于对照组,0.5hINS、1hINS、2hINS、0.5hCP、1hCP、2hCP均低于对照组,差异有统计学意义(P<0.05)。T2DM无并发症组、T2DM伴并发症组空腹PG、0.5hPG、1hPG、2hPG、3hPG均高于糖尿病前期组,0.5hINS、1hINS、0.5hCP、1hCP、2hCP均低于糖尿病前期组,T2DM伴并发症组2hINS低于糖尿病前期组,差异有统计学意义(P<0.05)。视网膜病变组1hPG低于酮症酸中毒组,1hCP、2hCP低于大血管病变组;大血管病变组2hPG低于酮症酸中毒组;周围神经病变组空腹PG、0.5hPG、2hPG、3hPG高于大血管病变组,差异有统计学意义(P<0.05)。结论T2DM患者葡萄糖负荷后CP的分泌在不同的并发症间存在差异,糖尿病早期即存在INS和CP分泌延迟和代偿性分泌增多。Objective To investigate the differences in serum levels of plasma glucose(PG),insulin(INS),and C-peptide(CP)in patients with type 2 diabetes mellitus(T2DM)after glucose load in Wuhan.Methods A total of 191 patients with T2DM admitted to Hubei Provincial Hospital of Traditional Chinese Medicine(hereinafter referred to as“our hospital”)from November 2020 to October 2021 were selected as the study group,and 74 non-diabetic patients admitted to our hospital during the same period were selected as the control group.After fasting for 8-10 h,75 g glucose was orally administered within five minutes in both groups.The levels of PG,INS,and CP at fasting and 0.5,1,2,and 3 h after glucose load were collected and compared.According to the different progression of T2DM,patients in the study group were divided into T2DM uncomplicated group(144 cases)and T2DM with complications group(47 cases).At the same time,21 patients diagnosed with prediabetes in the control group were selected as the prediabetes group.The differences of each detection index among the three groups were compared.In addition,according to the different complications of T2DM,the patients in T2DM with complications group were divided into peripheral neuropathy group(11 cases),retinopathy group(9 cases),ketoacidosis group(8 cases),macrovascular disease group(9 cases),and diabetic nephropathy group(10 cases).The differences of each detection index among the five groups were compared.Results Fasting PG,0.5hPG,1hPG,2hPG,3hPG in the study group were higher than those in the control group,and 0.5hINS,1hINS,2hINS,0.5hCP,1hCP,2hCP were lower than those in the control group,and the differences were statistically significant(P<0.05).Fasting PG,0.5hPG,1hPG,2hPG,and 3hPG in T2DM uncomplicated group and T2DM with complications group were higher than those in prediabetes group;0.5hINS,1hINS,0.5hCP,1hCP,and 2hCP were lower than those in prediabetes group;2hINS in T2DM with complications group were lower than those in prediabetes group,the differences were statistically si
