摘　　要：目的探讨赋能心理指导联合营养干预对原发肝癌经导管动脉化疗栓塞术(TACE)后患者康复的影响。方法通过随机抽样法选取2019年3月至2020年3月在江苏省人民医院肝脏五病区疼痛科实施TACE的原发性肝癌患者168例,利用随机数字表法将其分为观察组和对照组,各84例。对照组行常规护理,研究组在对照组基础上实践赋能心理指导联合营养干预护理,比较两组护理前后应对能力、负性情绪及营养状况。结果护理后,两组医学应对问卷量表中应对评分均升高,且观察组高于对照组,屈服、回避均降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组汉密尔顿抑郁量表、汉密尔顿焦虑量表评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组血红蛋白、上臂围、肱三头肌皮褶皱厚度均升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论将实践赋能心理指导联合营养干预护理应用在肝癌患者TACE术后患者中,提升其应对能力,减轻负性情绪,改善营养指标水平。Objective To explore the Effect of empowerment psychological guidance combined with nutritional intervention on rehabilitation of patients with primary liver cancer after transcatheter arterial chemoembolization(TACE).Methods A total of 168 patients with primary liver cancer who underwent TACE in the Department of Pain Liver Ward Fifth of Jiangsu Provincial People’s Hospital from March 2019 to March 2020 were selected.They were divided into observation group and control group by random number table method,with 84 cases in each group.The control group was received routine nursing,while the research group was received empowering psychological guidance combined with nutritional intervention nursing based on the control group.The coping ability,negative emotion,and nutritional status of the two groups were compared before and after nursing.Results After nursing,the coping score of the medical coping questionnaire in the two groups increased,and the observation group was higher than the control group,the yield and avoidance were decreased,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).The scores of Hamilton depression scale and Hamilton anxiety scale in the two groups were decreased,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).The hemoglobin,upper arm circumference and triceps skin fold thickness of the two groups were increased,and the observation group was higher than the control group,and the differences were statistically significant(P<0.05).Conclusion The empowering psychological guidance combined with nutritional intervention nursing was applied to patients with liver cancer after TACE to improve their coping ability,reduce negative emotions and improve the level of nutritional indicators.
