摘  要:目的探讨耐受评估指导营养管理应用在直肠造口患者中的价值。方法以随机抽样方法选取2019年12月至2020年12月在新疆医科大学第一附属医院实施直肠造口患者200例,利用随机数字表法将其分为试验组(100例)和对照组(100例)。试验组实施耐受评估指导营养管理,对照组实施常规营养护理干预。比较干预前、干预6周后两组营养状态改善、造口周围评估工具(DET)评分及胃肠功能状态。结果干预6周后,两组体重、体重指数、体脂率、体脂肪、握力高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。试验组造口周围皮肤改善情况优于对照组,差异有统计学意义(P<0.05)。试验组首次造口排气时间、首次造口排便时间、肠鸣音恢复时间及首次进食时间短于对照组,差异有统计学意义(P<0.05)。结论在直肠造口患者中应用耐受评估指导营养管理,改善机体营养状态,维持造口周围皮肤完整状态,促进胃肠功能恢复。Objective To explore the value of tolerance assessment to guide nutritional management in patients with rectostomy.Methods A random sampling method was used to select 200 patients with rectostomy in the First Affiliated Hospital of Xinjiang Medical University from December 2019 to December 2020.They were divided into experimental group(100 cases)and control group(100 cases)by random number table method.The experimental group was given tolerance assessment to guide nutritional management,while the control group was given routine nutritional care intervention.Before the intervention and after six weeks of intervention,the improvement of nutritional status,the discoloration,erosion,tissue overgronth(DET)score around the stoma and the gastrointestinal function status were compared between the two groups.Results After six weeks of intervention,the body weight,body mass index,body fat percentage,body fat,and grip strength in the two groups were higher than those before the intervention,while those of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The improvement of the skin around the stoma in the experimental group was better than that in the control group,and the difference was statistically significant(P<0.05).The time of the first stoma exhaust,the first stoma defecation time,the recovery time of bowel sounds,and the time of the first eating in the experimental group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Tolerance assessment is used to guide nutritional management in patients with rectostomy,improve the nutritional status of the body,maintain the integrity of the skin around the stoma,and promote the recovery of gastrointestinal function.
