PMIC-92和NMIC-413对分离自住院患者标本的细菌药敏检测性能

摘  要:目的评估革兰阳性细菌药敏板PMIC-92和革兰阴性细菌药敏板NMIC-413对临床常见细菌的药敏检测性能。方法选取从临床送检标本中分离的菌株40株(革兰阳性菌17株、革兰阴性菌23株)及实验室保存的标准株4株(大肠埃希菌ATCC25922、铜绿假单胞菌ATCC27853、金黄色葡萄球菌ATCC29213、粪肠球菌ATCC29212),分别采用PMIC-92(革兰阳性菌)和NMIC-413(革兰阴性菌)药敏板进行药敏测定,测定结果与实验室现有的VITEK2 Compact AST-GP639(革兰阳性球菌)、AST-GN13(肠杆菌科细菌)、AST-GN335(非发酵菌)检测结果进行比较,部分Vitek 2 Compact无法检测的药物则与K-B纸片扩散法结果进行对照,临床菌株每株检测1次,标准菌株每日检测3次,连续5 d,各测15次。结果PMIC-92药敏板共比对了14种抗菌药物,总分类一致性(CA)为99.08%,小误差(MIE)率为0.37%,重大差异(MD)率为0.55%,无极重大差异(VMD);NMIC-413药敏板共比对了17种抗菌药物,总CA为97.65%,MIE率为1.03%,MD率为1.17%,VMD率为0.15%。结论药敏板PMIC-92和NMIC-413能分别对临床分离的革兰阳性菌和革兰阴性菌的药物敏感性进行快速、准确的测定,可应用于临床实验室进行药敏检测。Objective To evaluate antibiotic sensitive plate of Gram-positive bactreia PMIC-92 and Gram-negative bactreia NMIC-413 for antimicrobial susceptibility testing(AST)of clinically relevant bacteria.Methods Forty clinical strains,including 17 Gram-positive strains and 23 Gram-negative strains,and 4 type strains of AST kept in our laboratory,including Escherichia coli ATCC25922,Pseudomonas aeruginosa ATCC27853,Staphylococcus aureus ATCC29213,and Enterococcus faecalis ATCC29212,were studied.The AST of these strains were detected by BD Phoenix M50 with PMIC-92(for Gram-positive bacteria)and NMIC-413(for Gram-negative bacteria).VITEK 2 Compact with the respective AST card GP639 for gram-positive coccus,GN13 for enterobacteriaceae,GN335 for non-fermenting gram-negative bacilli(NFGNB)were used as the references for comparison.Some antibiotics could not tested by Vitek 2 accurately,and then were detected by K-B disk diffusion method.Each clinical strain was tested once and each type strain was tested fifteen times(three times a day,five consecutive days).Results Fourteen antibiotics of PMIC-92 were evaluated with the total classification consistency(CA)of 99.08%,the minor error(MIE)of 0.37%,and the major discrepancies(MD)of0.55%;no very major discrepancies(VMD)were found.Seventeen antibiotics of NMIC-413 were evaluated and the total CA was 97.65%,MIE was 1.03%,MD was 1.17%,and VMD was 0.15%.Conclusion Antibiotic sensitive plates PMIC-92 and NMIC-413 can rapidly and accurately determine the drug susceptibility of clinically isolated Gram-positive and Gram-negative bacteria,respectively,which can be applied in clinical laboratory for AST.
