摘 要:目的探讨超声引导在输尿管软镜下钬激光内开窗治疗肾盂旁囊肿的价值。方法回顾性分析2017年3月至2020年8月在福建省石狮市医院泌尿外科接受输尿管软镜下钬激光内开窗治疗的29例肾盂旁囊肿患者的临床资料,其中21例通过输尿管软镜探查成功定位囊肿(初始成功组);8例患者定位失败(初始失败组),对于初始失败病例,通过超声及内镜联合定位后成功寻及囊肿。结果2组病例的手术总体时间、住院时间比较均差异无统计学意义(P>0.05)。初始失败组在应用超声后的囊肿定位时间[(3.5±1.0)min]明显短于初始成功组[(11.8±7.9)min],差异有统计学意义(P<0.05)。术后随访期间,CT复查结果显示,初始成功组和初始失败组的影像学有效率分别为95%和100%。结论对于定位困难的肾盂旁囊肿病例,超声联合内镜定位能明显缩短定位时间,提高囊肿定位成功率,更安全可靠。Objective To evaluate the value of holmium laser fenestration under ultrasound-guided flexible ureteroscope in treatment of parapelvic cysts.Methods The clinical data of 29 patients with parapelvic cysts who received holmium laser fenestration under flexible ureteroscope from March 2017 to August 2020 were retrospectively analyzed.Of all the cases,21 were successfully localized by flexible ureteroscope(designated as the initial successful group)and the other 8 cases failed to localize cysts(designated as the initial failure group).For those initial failure cases,cysts were then successfully localized by ultrasound and ureteroscope.Results There were no significant differences in total operation time and hospitalization time,when comparisons were made between the2 groups(P>0.05).The cyst localization time(3.5±1.0)min by applying ultrasound in the initial failure group was significantly shorter than that in the initial success group(11.8±7.9)min(P<0.05),with statistical significance.During the time of postoperative follow-up,CT reexamination showed that CT imaging effective rates were respectively 95%and 100%in the initial success group and the initial failure group.Conclusion For those cases hard to localize parapelvic cysts,the combination of ultrasound and ureteroscope could significantly shorten the localization time and improve the success rate of cyst localization.Ultrasonic real-time monitoring of cyst incision and catheter drainage is safer and more reliable.
