不同剂量纳布啡联合舒芬太尼在妇科腹腔镜手术术后镇痛的安全性分析

摘　　要：目的:分析不同剂量纳布啡联合舒芬太尼在妇科腹腔镜手术术后镇痛的安全性。方法:选取2019年10月—2021年10月我院135例行妇科腹腔镜手术的患者作为研究对象,根据术后镇痛泵用药剂量不同分为甲组、乙组和丙组,每组45例。甲组给予纳布啡1.0 mg/kg+舒芬太尼1.5μg/kg,乙组给予纳布啡1.5 mg/kg+舒芬太尼1.0μg/kg,丙组给予纳布啡2.0 mg/kg+舒芬太尼0.5μg/kg,比较三组疼痛程度、患者满意度及不良反应发生率。结果:三组术后2、4 h VAS评分比较,差异无统计学意义(P>0.05);三组术后8、12、24、48 h VAS评分比较,差异有统计学意义(P<0.05)。乙组满意度高于甲、丙组,差异有统计学意义(P<0.05)。乙组不良反应发生率低于甲、丙组,差异有统计学意义(P<0.05)。结论:纳布啡1.5 mg/kg+舒芬太尼1.0μg/kg在妇科腹腔镜手术中可降低患者疼痛感,且镇静效果好,不良反应发生率低,患者满意度较高。Objective:To analyze the safety of different doses of nalbuphine combined with sufentanil in postoperative analgesia of gynecological laparoscopic surgery.Methods:A total of 135 patients who underwent gynecological laparoscopic surgery in our hospital from October 2019 to October 2021 were selected as subjects.According to the different doses of postoperative analgesia pump,they were divided into group A,group B and group C,with 45 cases in each group.Group A was given nalbuphine 1.0 mg/kg+sufentanil 1.5μg/kg,group B was given nalbuphine 1.5 mg/kg+sufentanil 1.0μg/kg,group C was given nalbuphine 2.0 mg/kg+sufentanil 0.5μg/kg.The degree of pain,patient satisfaction and incidence of adverse reactions were compared among the three groups.Results:There was no significant difference in VAS scores between the three groups at 2 and 4 h after operation(P>0.05).The VAS scores at 8,12,24 and 48 h after operation in the three groups were compared,and the difference was statistically significant(P<0.05).The satisfaction of group B was higher than that of group A and group C,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in group B was lower than that in group A and C,and the difference was statistically significant(P<0.05).Conclusion:Nabuphine 1.5 mg/kg+sufentanil 1.0μg/kg can reduce the pain of patients in gynecological laparoscopic surgery,and has good sedative effect,low incidence of adverse reactions,and high patient satisfaction.
