摘  要:目的:观察补肺益肾组分方、针刺及其联合在不同干预时间对慢性阻塞性肺疾病(COPD)大鼠的疗效特点。方法:240只SD大鼠随机分为正常组、模型组、氨茶碱组、补肺益肾组分方组(组分方组)、针刺组、补肺益肾组分方联合针刺组(联合组),1—12周卷烟烟雾熏吸联合细菌感染法制备COPD稳定期大鼠模型。13—20周各治疗组分别给予不同干预手段,停止干预后继续观察至第28周,分别于14、16、20、24、28周取材。检测肺功能[用力肺活量(FVC)、第0.1秒用力呼气容积(FEV0.1)]、肺病理[肺泡平均截距(MLI)、平均肺泡数(MAN)、支气管管壁厚度(Wt)]和肺组织氧化应激相关指标[总超氧化物歧化酶(T-SOD)、总抗氧化能力(T-AOC)、脂质过氧化物(LPO)],评价不同干预手段对COPD大鼠的疗效特点。结果:肺功能:各干预组第16周升高FVC和FEV0.1(P<0.01),其中,组分方组和氨茶碱组疗效持续至第24周(P<0.05,P<0.01),针刺组至第20周(P<0.05),联合组至第28周且优于组分方组和针刺组(P<0.05,P<0.01)。肺病理:各干预组均可升高MAN,降低MLI和Wt,其中,联合组起效早且持续至第28周(P<0.05,P<0.01)。氧化应激:各治疗组均可升高T-SOD、T-AOC,降低LPO,其中,联合组可持续升高T-SOD、T-AOC至第28周,降低LPO持续至第24周(P<0.05,P<0.01)。结论:各干预方案对COPD大鼠均有良好疗效,其中,组分方组和联合组起效时间早,联合组远后效应最佳。Objective: To observe the therapeutic characteristics of effective-component compatibility of Bufei Yishen Formula,acupuncture and its combination in the treatment of chronic obstructive pulmonary disease(COPD) rats at different intervention time.Methods: A total of 240 SD rats were randomly divided into normal, model, aminophylline(APL), effective-component compatibility of Bufei Yishen Formula(ECC-BYF), acupuncture, and ECC-BYF combined with acupuncture(Combination) groups, The COPD stable phase rat model was prepared by cigarette smoke exposure and bacterial infection during 1 to 12 weeks, from 13th week to 20th week,rats were given different interventions. After stopping the intervention, rats were observed until the 28th week. The rats were sacrificed at 14th, 16th, 20th, 24th and 28th week. Lung function(FVC, FEV0.1), lung pathology(MLI, MAN, Wt) and oxidative stress index contents(T-SOD, T-AOC, LPO) in lung tissue were detected to evaluate the curative effect of different interventions in COPD rats. Results: Lung function: FVC and FEV0.1 were increased in each group at 16th week(P<0.01), thereinto, the efficacy of the group of ECC-BYF and APL lasted until 24th week(P<0.05, P<0.01), the efficacy of the group of acupuncture lasted until 20th week(P<0.05), the efficacy of the group of Combination lasted until 28th week and superior to the group of ECC-BYF and acupuncture(P<0.05, P<0.01). Lung pathology: MAN was increased, MLI and WT were deceased in all the intervention groups. The onset time of the Combination group was early and lasted until the 28th week(P<0.05,P<0.01). Oxidative stress: T-SOD and T-AOC were increased, LPO was decreased in all the intervention groups. T-SOD and T-AOC was increased until 28th week in the Combination group, and LPO was decreased until 24th week(P<0.05,P<0.01).
