摘 要:目的:观察砭贴预处理对急性低压缺氧肺水肿(即高原肺水肿)大鼠肺组织损伤的保护作用及其对NF-κB及相关炎症因子表达的影响。方法:将32只雄性SD大鼠随机分成正常组、模型组、砭贴组、药物组,每组8只。正常组和模型组正常饲养,砭贴组于太渊、内关、人迎穴每天贴砭贴2h,药物组用大株红景天胶囊按280 mg/kg的剂量每天灌胃1次,均预处理14 d。除正常组外,其余3组于第15天置于低压低氧实验舱24 h建立急性低压缺氧肺水肿大鼠模型(即高原肺水肿模型)。出舱后采集腹主动脉血及肺组织,HE染色观察肺组织病理变化,测定肺含水量(LWC),血气分析仪检测血液酸碱度(pH)、动脉血二氧化碳分压(PaCO)、动脉血氧气分压(PaO),ELISA法测定肺组织肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-1β水平,RT-PCR检测肺组织核转录因子-κB p65(NF-κB p65)mRNA表达。结果:与正常组比较,模型组肺组织损伤明显,LWC显著增加(P<0.01),血液pH、PaO、PaCO均显著降低(P<0.01),肺组织中TNF-α、IL-6、IL-1β水平显著升高(P<0.01),NF-κB p65 mRNA表达显著升高(P<0.01);与模型组比较,砭贴组与药物组肺组织损伤减轻,LWC显著降低(P<0.01),血液pH、PaO、PaCO显著升高(P<0.01),肺组织中TNF-α、IL-6、IL-1β水平显著降低(P<0.05,P<0.01),NF-κB p65 mRNA表达显著降低(P<0.05,P<0.01)。结论:砭贴预处理对急性低压缺氧肺水肿大鼠有明显保护及抗炎作用,其作用机制可能是通过下调NF-κB p65 mRNA表达,减少炎症因子的释放,从而抑制炎性损伤。Objective: To observe the protective effects of plaster acupuncture pretreatment on lung tissue injury and the effects of NF-κB and related inflammatory factors expression in rats with acute low-pressure hypoxic pulmonary edema.Methods: Thirty-two male SD rats were randomly divided into normal group, model group, plaster acupuncture group and drug group, 8 rats in each group. The normal group and the model group were fed normally, the plaster acupuncture group applied plaster acupuncture paste at Taiyuan(LU 9), Neiguan(PC 6) and Renying(ST 9) points for 2 h daily, and the drug group used Rhodiola Rosea Capsules at a dose of 280 mg/kg once daily by gavage, all pretreated for 14 d. Except for the normal group,the other three groups were placed in a low-pressure oxygen chamber for 24 h on the 15th day. Abdominal aortic blood and lung tissue were collected after exiting the chamber. The pathological changes of lung tissue were observed by HE staining,lung water content(LWC) was measured, blood pH, PaOand PaCOwere measured by blood gas analyzer, TNF-α, IL-6 and IL-1β levels in lung tissue were measured by ELISA, and NF-κB p65 mRNA expression in lung tissue was detected by RT-PCR.Results: Compared with the normal group, the lung tissue of the model group was significantly damaged, LWC was significantly increased(P<0.01), the blood pH, PaOand PaCOwere decreased(P<0.01), the levels of TNF-α, IL-6 and IL-1β in the lung tissue were significantly increased(P<0.01), and the expression of NF-κB p65 mRNA was increased(P<0.01). Compared with the model group, the lung tissue damages in the plaster acupuncture group and drug group were alleviated, LWC was decreased(P<0.01), the blood pH, PaO, and PaCOwere significantly increased(P<0.01), the levels of TNF-α, IL-6, and IL-1β in the lung tissue were significantly decreased(P<0.05, P<0.01), and the expression of NF-κB p65 mRNA was decreased(P<0.05, P<0.01).Conclusion: The plaster acupuncture pretreatment had a significant protective effect and anti-inflammatory effect on a
