自体动静脉瘘术后功能障碍的危险因素分析

摘　　要：目的探究自体动静脉瘘术后功能障碍的危险因素。方法回顾性收集2015~2020年于肇庆市第一人民医院血管外科行动静脉内瘘成形术的274例患者的临床资料,使用多因素Logistic回归分析术后功能障碍的独立危险因素,并绘制列线图模型。结果患者术后平均随访时间为(31.1±7.2)个月,85例患者(31.0%)出现动静脉内瘘功能障碍,出现功能障碍的平均时间为(22.6±6.8)个月。多因素Logistics回归分析结果显示,年龄(OR=1.451,95%CI=1.123~1.672)、低密度脂蛋白(OR=1.315,95%CI=1.026~1.688)和吸烟史(OR=1.513,95%CI=1.221~1.709)是术后功能障碍的独立危险因素。以此建立的列线图风险模型能早期识别高危患者,早期进行干预。结论年龄、低密度脂蛋白、吸烟是自体动静脉瘘术后功能障碍的独立危险因素。Objective To explore the risk factors of postoperative autologous arteriovenous fistula dysfunction.Methods The clinical data of 274 patients who underwent intravenous fistula angioplasty in the Department of Vascular Surgery of the First People's Hospital of Zhaoqing between 2015 and 2020 were analyzed retrospectively.Multivariate logistic regression was used to analyze independent risk factors for postoperative dysfunction,and a nomogram model was drawn.Results The average follow-up time was(31.1±7.2)months.During the follow-up period,85 patients(31.0%)developed autogenous arteriovenous fistula dysfunction,and the average time to appear dysfunction was(22.6±6.8)months.The results of multivariate logistic regression showed that age(OR=1.451,95%CI=1.123~1.672),low-density lipoprotein(OR=1.315,95%CI=1.026~1.688),and smoking history(OR=1.513,95%CI=1.221~1.709)were independent risk factors for postoperative dysfunction.Based on the selected independent risk factors,a predictive nomogram model was established to identify high-risk patients early and intervene early.Conclusion Age,low-density lipoprotein and smoking history are independent risk factors for dysfunction after autologous arteriovenous fistula surgery.
