国内外精益医疗在优化医院服务流程中的应用现状述评

摘　　要：目的 概述国内外精益医疗在医院服务流程中的应用,为精益医疗在医院服务流程的进一步实践和理论研究提供参考。方法 在中国知网(CNKI)、维普、超星、万方等数据库,以“主题”含“精益”“医院”为检索策略,同时在SciFinder、SpringerLink、Embase和PubMed等数据库与Endnote软件中,以“Title”含“Lean”为检索策略,描述文献发表的一般情况。结果 共检索到257篇文献,26篇文献纳入分析,其中外文文献15篇,中文文献11篇。医院流程优化关注的热点区域集中在手术室(19.23%)和药房(19.23%)。精益工具的使用集中在价值流图(57.69%)、目视化管理(38.46%)和5S方法(38.46%)。服务流程优化的精益项目中最常使用的测量指标类别是比率类指标。结论 国外的研究范围更广泛且深入,使用的精益工具和测量指标更加多样化;国内研究领域较为局限浅显,并且精益工具和指标的运用均比较单一。国内医院应大胆探索精益医疗改善范围,积极学习及运用精益思维和多种精益工具,深入探究精益医疗开展领域的影响效果。Objective The application of lean medical treatment in hospital service process at home and abroad was summarized in order to provide reference for further practice and theoretical research of lean medical treatment in hospital service process.Methods The retrieval strategy of "subject" including "Lean" and "hospital" was used in CNKI,Weipu, Super Star, Wanfang and other databases, and the retrieval strategy of "Title" and "Lean" was used in SciFinder, SpringerLink, Embase, PubMed and other databases and Endnote software.Results A total of 257 literatures were retrieved and 26 literatures were included in the analysis, including 15 literatures in foreign languages and 11 literatures in Chinese.Hospital process optimization focuses on the operating room(19.23%)and pharmacy(19.23%).Lean tools were used in value stream maps(57.69%),visual management(38.46%),and the 5 S method(38.46%).The most commonly used category of metrics in lean projects for service process optimization was the ratio category.Conclusion The research scope of foreign countries is more extensive and in-depth, and the use of lean tools and measurement indicators is more diversified.The domestic research field is relatively limited and shallow, and the application of lean tools and indicators is relatively simple.Domestic hospitals should boldly explore the scope of lean medical improvement, actively learn and use lean thinking and various lean tools, and deeply explore the impact of lean medical development.
