保守治疗结合中医调理脾肾阳虚法治疗急性腰椎间盘突出症的效果评价

摘　　要：目的:探讨中医调理脾肾阳虚法辅助治疗急性腰椎间盘突出症(LIDP)的临床效果。方法:选取2019年2月-2021年1月厦门市中医院收治的134例急性LIDP患者,按照随机数字表法将其分为对照组(n=67)和观察组(n=67)。对照组给予常规保守治疗,观察组在对照组基础上给予中医调理脾肾阳虚法辅助治疗。比较两组主要症状改善情况、腰椎关节活动度、Oswestry腰椎功能障碍指数(ODI)评分、日本骨科协会评估治疗分数(JOA)、视觉模拟评分量表(VAS)评分、神经传导速度、血清炎症因子水平。结果:治疗2、4周,观察组主要症状评分均低于对照组,腰椎前屈和后伸活动度均高于对照组(P<0.05)。治疗2、4周,观察组ODI、VAS评分均低于对照组,JOA评分均高于对照组(P<0.05)。治疗2、4周,观察组腓浅神经和腓肠神经的感觉传导速度(SCV)与腓神经和胫神经运动的传导速度(MCV)均低于对照组(P<0.05)。治疗2、4周,观察组血清肿瘤坏死因子-α(TNF-α)、金属基质蛋白酶-3(MMP-3)、单核细胞趋化蛋白-1(MCP-1)水平均低于对照组(P<0.05)。结论:中医调理脾肾阳虚法辅助保守治疗可有效减轻炎症,减轻疼痛,促进急性LIDP患者腰椎功能恢复。Objective:To explore the clinical efficacy of traditional Chinese medicine to regulate spleen and kidney yang deficiency in the treatment of acute lumbar intervertebral disc herniation(LIDP).Method:A total of 134 patients with acute LIDP in Xiamen Hospital of Traditional Chinese Medicine from February 2019 to January 2021 were selected,they were divided into control group(n=67)and observation group(n=67)according to random number table method.The control group was given conventional conservative treatment,and the observation group was given traditional Chinese medicine to regulate spleen and kidney yang deficiency on the basis of the control group.Symptom improvement,lumbar joint range of motion,oswestry lumbar dysfunction index(ODI)scores,Japanese orthopaedic association evaluation treatment scores(JOA),visual analog scale(VAS)scores,nerve conduction velocity and serum inflammatory factors were compared between two groups.Result:After 2 and 4 weeks of treatment,the main symptom scores of the observation group were lower than those of the control group,and the lumbar flexion and extension motion range of the observation group was higher than that of the control group(P<0.05).After 2 and 4 weeks of treatment,the ODI and VAS scores of the observation group were lower than those of the control group,and the JOA scores of the observation group were higher than those of the control group(P<0.05).After 2 and 4 weeks of treatment,the sensory conduction velocity(SCV)of superficial peroneal nerve and sural nerve and the motor conduction velocity(MCV)of peroneal nerve and tibial nerve of the observation group were lower than those of the control group(P<0.05).After 2 and 4 weeks of treatment,the serum levels of tumor necrosis factor-α(TNF-α),matrix metalloproteinase-3(MMP-3)and monocyte chemoattractant protein-1(MCP-1)of the observation group were lower than those of the control group(P<0.05).Conclusion:The traditional Chinese medicine to regulate spleen and kidney yang deficiency assisted conservative treatment can effe
