逐瘀通窍聪耳汤联合穴位注射治疗气滞血瘀型暴聋的临床观察

摘　　要：目的:研究逐瘀通窍聪耳汤联合穴位注射治疗气滞血瘀型暴聋的临床效果。方法:选取2019年6月-2021年6月淮安市中医院收治的68例气滞血瘀型暴聋患者,采用随机数字表法将其分为对照组和观察组,各34例。两组除接受统一的西药治疗外,对照组另行穴位注射,观察组在对照组的基础上予以逐瘀通窍聪耳汤。比较两组疗效、0.25、0.5、1、2、4、8 kHz频率的听阈值、血液流变学指标[全血黏度(WBV)、血细胞比容(Hct)、血浆黏度(PV)]、症状消失时间及听力恢复时间。结果:观察组总有效率为91.18%,高于对照组的70.59%(P<0.05)。治疗后,观察组各频率听阈值、WBV、Hct、PV均低于对照组(P<0.05)。观察组耳闷、耳鸣、眩晕消失时间及听力恢复时间均短于对照组,差异均有统计学意义(P<0.05)。结论:逐瘀通窍聪耳汤联合穴位注射能够缓解气滞血瘀型暴聋患者耳鸣、耳闷及眩晕等症状,改善其听力以及血液流变学情况,效果确切。Objective:To study the clinical effect of Zhuyu Tongqiao Cong’er Decoction combined with acupoint injection in the treatment of deafness caused by qi stagnation and blood stasis type.Method:A total of 68 patients with qi stagnation and blood stasis type deafness admitted in Huai’an Hospital of Traditional Chinese Medicine from June 2019 to June 2021 were selected,they were divided into control group and observation group according to random number table,with 34 cases in each group.In addition to the unified western medicine treatment,the control group was given acupoint injection,and the observation group was given Zhuyu Tongqiao Cong’er Decoction on the basis of the control group.The curative effect,hearing threshold of 0.25,0.5,1,2,4,8 kHz frequency,hemorheological indexes[whole blood viscosity(WBV),hematocrit(Hct),plasma viscosity(PV)],symptom disappearance time and hearing recovery time were compared between the two groups.Result:The total effective rate of the observation group was 91.18%,which was higher than 70.59%of the control group(P<0.05).After treatment,the average hearing threshold of each frequency,WBV,Hct and PV of the observation group were lower than those of the control group(P<0.05).The disappearance time of ear fullness,tinnitus,vertigo and hearing recovery time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).Conclusion:Zhuyu Tongqiao Cong’er Decoction combined with acupoint injection can relieve the symptoms of tinnitus,ear fullness and dizziness in patients with Qi stagnation and blood stasis type deafness,and improve their hearing and blood rheology.The effect is definite.
