闭合复位联合经皮交叉克氏针固定术治疗小儿肱骨髁上骨折的临床观察

摘　　要：目的:探讨闭合复位联合经皮交叉克氏针固定术治疗小儿肱骨髁上骨折的临床效果。方法:选择2019年6月-2021年6月佳木斯市中心医院收治的110例小儿肱骨髁上骨折患儿为手术治疗对象。随机分为观察组和对照组,各55例。对照组以切开复位克氏针内固定治疗,观察组以闭合复位联合经皮交叉克氏针固定术治疗。比较两组手术相关指标、骨折愈合时间、并发症发生情况、手术优良率、肘关节活动度和生活质量评分。结果:观察组手术时间、住院时间及骨折愈合时间均短于对照组,术中失血量少于对照组(P<0.05)。观察组并发症发生率为7.27%(4/55),低于对照组的21.82%(12/55),差异有统计学意义(P<0.05)。观察组手术优良率为90.91%(50/55),高于对照组的72.73%(40/55),差异有统计学意义(P<0.05)。术后,观察组肘关节旋前、旋后、背伸、屈曲活动度均高于对照组(P<0.05)。术后,观察组儿童少年生活质量量表(QLSCA)各维度评分均高于对照组(P<0.05)。结论:闭合复位联合经皮交叉克氏针固定术治疗小儿肱骨髁上骨折的临床效果显著,能够促进患儿骨折愈合,提高手术优良率和肘关节活动功能,且并发症发生率低,可改善患儿的生活质量,值得临床推荐。Objective:To study the clinical effect of closed reduction combined with percutaneous cross Kirschner wire fixation in the treatment of supracondylar fracture of humerus in children.Method:A total of 110 children with supracondylar fracture of humerus in Jiamusi Central Hospital from June 2019 to June 2021 were selected for surgical treatment.They were randomly divided into observation group and control group,55 cases in each group.The control group was treated with open reduction and Kirschner wire fixation,and the observation group was treated with closed reduction and percutaneous cross Kirschner wire fixation.Operation related indexes,fracture healing time,complications,excellent rate of operation,elbow range of motion and quality of life score were compared between the two groups.Result:The operative time,hospital stay and fracture healing time in observation group were shorter than those in control group,intraoperative blood loss was less than that in control group(P<0.05).The complication rate in the observation group was 7.27%(4/55),which was lower than 21.82%(12/55)in the control group,the difference was statistically significant(P<0.05).The excellent and good rate of the observation group was 90.91%(50/55),which was higher than 72.73%(40/55)of the control group,the difference was statistically significant(P<0.05).After operation,the activity of elbow pronation,supination,dorsiflexion and flexion in the observation group were higher than those in the control group(P<0.05).After operation,the scores of each dimension of the quality of life scale for children and adolescents(QLSCA)in the observation group were higher than those in the control group(P<0.05).Conclusion:Closed reduction combined with percutaneous cross Kirschner wire fixation is clinically significant in the treatment of supracondylar fractures of humerus in children.It can promote the rapid healing of fractures in children,improve the excellent rate of surgical reduction and fixation and elbow movement function,and has a low incidence of com
