MIPPO技术联合锁定加压钢板治疗胫骨下端骨折的临床研究

摘　　要：目的:探讨微创经皮钢板内固定(MIPPO)技术联合锁定加压钢板治疗胫骨下端骨折的效果。方法:回顾性分析2018年1月-2021年8月福建医科大学龙岩第一附属医院骨科收治的260例胫骨下端骨折患者的临床资料,根据不同手术方式分为对照组(n=130)和观察组(n=130)。对照组使用单一钢板内固定治疗,观察组使用MIPPO技术联合锁定加压钢板治疗。比较两组疗效、手术情况、住院时间、Rasmussen膝关节、Baird-Jackson踝关节功能、膝关节功能(HSS)评分及并发症发生情况。结果:观察组总有效率为94.62%,高于对照组的80.00%(P<0.05)。观察组术中出血量少于对照组,手术时间、骨痂出现时间及住院时间均短于对照组(P<0.05)。术后3个月,观察组Rasmussen膝关节、Baird-Jackson踝关节功能评分、HSS评分均高于对照组(P<0.05)。对照组并发症发生率为11.54%,高于观察组的0.77%(P<0.05)。结论:MIPPO技术联合锁定加压钢板治疗胫骨下端骨折效果较好,可减少术中出血量,对患者骨折愈合有积极作用。Objective:To investigate the effect of minimal invasive percutaneous plate osteosynthesis(MIPPO)combined with locking compression plate in the treatment of lower tibial fracture.Method:The clinical data of 260 patients with lower tibial fracture admitted to Department of Orthopedics,Longyan First Hospital Affiliated to Fujian Medical University from January 2018 to August 2021 were retrospectively analyzed,according to different surgical methods,they were divided into control group(n=130)and observation group(n=130).The control group was treated with single plate internal fixation,while the observation group was treated with MIPPO technology combined with locking compression plate.The efficacy,operation situation,length of hospital stay,Rasmussen knee joint scores,Baird-Jackson ankle function scores,knee joint function(HSS)scores and complications were compared between the two groups.Result:The total effective rate of the observation group was 94.62%,which was higher than 80.00%of the control group(P<0.05).The intraoperative blood loss of the observation group was less than that of the control group,and the operation time,callus appearance time and hospital stay of the observation group were shorter than those of the control group(P<0.05).3 months after operation,the Rasmussen knee joint scores,Baird-Jackson ankle joint function scores and HSS scores of the observation group were higher than those of the control group(P<0.05).The incidence of complications of the control group was 11.54%,which was higher than 0.77%of the observation group(P<0.05).Conclusion:MIPPO combined with locking compression plate is effective in the treatment of lower tibial fracture,which can reduce intraoperative blood loss and has a positive effect on fracture healing.
