支气管热成形术联合射麻止喘液对重症哮喘患者的疗效及生化指标的影响

摘　　要：目的研究支气管热成形术(bronchial thermoplasty,BT)联合射麻止喘液对重症哮喘患者的疗效及生化指标的影响。方法选取2017年12月-2020年1月中国科学院大学深圳医院收治的重症哮喘患者60例,按照信封法随机等分为联合组及常规组,各30例。常规组实施沙美特罗替卡松粉吸入剂(ICS-LABA)治疗,联合组实施BT联合射麻止喘液治疗。比较两组临床疗效、治疗前后肺功能、血清及肺泡灌洗液(BALF)中IL 17、TGF-β水平的变化情况。结果联合组总有效率(96.67%)明显高于常规组(76.67%),差异有统计学意义(χ^(2)=5.192,P<0.05)。治疗后,两组FVC、FEV1、FEV1/FVC水平对比,差异无统计学意义(P>0.05)。治疗后,联合组血清及BALF中的IL-17、TGF-β水平均显著低于常规组,差异有统计学意义(P<0.05)。结论BT联合射麻止喘液可改善重症哮喘患者血清及BALF中的IL-17、TGF-β水平,疗效显著。Objective To study the curative effect and biochemical indexes of bronchial thermoplasty(BT)combined with Shema Zhichuan Liquid on severe asthma patients.Methods A total of 60 patients with severe asthma admitted to Shenzhen Hospital,University of Chinese Academy of Sciences from December 2017 to January 2020 were selected and randomly divided into combination group and conventional group according to the envelope method,with 30 cases in each group.The routine group was treated with salmeterol-ticasone powder inhalation(ICS-LABA),and the combination group was treated with BT Combined with Shema Zhichuan liquid.The clinical efficacy,the changes of pulmonary function,serum and bronchoalveolar lavage fluid(BALF)levels of IL-17 and TGF-βbefore and after treatment were compared between the two groups.Results The curative effect of the combined group(96.67%)was significantly higher than that of the conventional group(76.67%),and the difference was statistically significant(χ^(2)=5.192,P<0.05).There was no statistically significant difference in the levels of FVC,FEV1,and FEV1/FVC between the two groups after treatment(P>0.05).After treatment,the levels of IL-17 and TGF-βin serum and BALF of the combined group were significantly lower than those of the conventional group,and the difference was statistically significant(P<0.05).Conclusion BT combined with Shema Zhichuan Liquid can improve the levels of IL-17 and TGF-βin serum and BALF in patients with severe asthma,and the therapeutic effect is significant.
