清肿消散汤联用迈之灵防治LIHR后血清肿的效果分析

摘　　要：目的探讨口服清肿消散汤与迈之灵在预防和治疗腹腔镜腹股沟疝修补术(laparoscopic inguinal hernia repair,LIHR)后血清肿中的作用。方法选取2016年6月-2019年11月在庆阳市西峰区人民医院行LIHR手术的腹股沟疝患者160例,随机分为两组,各80例。对照组LIHR术后给予基础治疗以防血清肿,观察组LIHR术后加服清肿消散汤与迈之灵治疗。术后14 d记录血清肿发生情况和疼痛评分。结果观察组术后14 d血清肿发生率(7.5%)显著少于对照组(25.0%),差异有统计学意义(χ^(2)=9.001,P=0.003);观察组血清肿体积(0.49±0.21)cm^(3)小于对照组(2.14±0.67)cm^(3),差异有统计学意义(t=21.019,P<0.001)。术后两组血清肿分型均以Ⅰ~Ⅱ型为主,两组血清肿分型比较,差异无统计学意义(P>0.05)。观察组术后各时段疼痛评分(2.7±0.8)分、(2.4±0.3)分、(2.1±0.4)分、(1.3±0.3)分低于对照组的(3.6±0.9)分、(3.0±0.5)分、(2.7±0.8)分、(1.8±0.4)分,差异有统计学意义(P<0.05)。结论口服清肿消散汤与迈之灵能有效预防LIHR术后血清肿发生,缩小血清肿体积,促进血清肿消退,减轻术区疼痛。Objective To investigate the effect of oral administration of Qingzhongxiaosan decoction and Maizhiling in the prevention and treatment of seroma after laparoscopic inguinal hernia repair(LIHR).Methods A total of 160 patients with inguinal hernia who underwent LIHR surgery in Xifeng District People's Hospital of Qingyang City from June 2016 to November 2019 were selected and randomly divided into two groups,with 80 cases in each group.The control group was given basic treatment to prevent seroma after LIHR,and the observation group was given Qingzhongxiaosan decoction and Maizhiling after LIHR.The occurrence of seroma and pain score were recorded 14 d after operation.Results The incidence of seroma in the observation group(7.5%)was significantly lower than that in the control group(25.0%)at 14 d after operation,and the difference was statistically significant(χ^(2)=9.001,P=0.003).The volume of seroma(0.49±0.21)cm^(3) in the observation group was smaller than that of the control group(2.14±0.67)cm^(3),and the difference was statistically significant(t=21.019,P<0.001).The type of serum swelling in the two groups after operation was mainly type I~II,and there was no statistically significant difference between the two groups(P>0.05).The pain scores in each period after operation(2.7±0.8)points,(2.4±0.3)points,(2.1±0.4)points,(1.3±0.3)points were lower than those of the control group(3.6±0.9)points,(3.0±0.5)points,(2.7±0.8)points,(1.8±0.4)points,and the difference was statistically significant(P<0.05).Conclusion Oral administration of Qingzhongxiaosan decoction and Maizhiling can effectively prevent the occurrence of seroma after LIHR,reduce the volume of seroma,promote the regression of seroma,and relieve pain in the operation area.
