摘 要:目的探讨胰岛素泵持续皮下注射与胰岛素静脉滴注两种方式在治疗儿童糖尿病并酮症酸中毒中的有效性及安全性。方法选取2020年12月-2021年9月甘肃河西学院附属张掖人民医院收治的80例儿童糖尿病并酮症酸中毒患者为研究对象,按照治疗方式不同分为治疗组和对照组,每组40例。治疗组给予胰岛素泵持续皮下注射治疗,对照组给予胰岛素静脉滴注治疗,比较两组患儿血酮转阴用时、尿酮转阴用时、血糖达标用时、胰岛素用量、治疗有效率及并发症发生率。结果治疗组患儿的血酮转阴用时及尿酮转阴用时短于对照组,差异有统计学意义(t=22.866、25.191,P<0.05)。治疗组患儿血糖达标用时及胰岛素用量优于对照组,差异有统计学意义(t=6.302、13.828,P<0.05),治疗组患儿总有效率高于对照组,差异有统计学意义(χ^(2)=4.114,P<0.05)。治疗组患儿并发症发生率5.00%低于对照组的20.00%,差异有统计学意义(χ^(2)=4.114,P<0.05)。结论儿童糖尿病并酮症酸中毒治疗中给予胰岛素泵持续皮下注射治疗的效果要好于胰岛素静脉滴注治疗。Objective To investigate the efficacy and safety of continuous subcutaneous injection of insulin pump and intravenous infusion of insulin in the treatment of children with diabetes mellitus and ketoacidosis.Methods A total of 80 children with diabetes and ketoacidosis who were admitted to Zhangye People's Hospital Affiliated to Hexi University in Gansu Province from December 2020 to September 2021 were selected as the research objects and divided into a treatment group and a control group according to the different treatment methods,40 cases in each group.The children in the treatment group were given continuous subcutaneous injection of insulin pump,and the control group was given intravenous insulin drip therapy.Compared the time of blood ketone turning negative,urine ketone turning negative,blood glucose reaching the standard,insulin dosage,treatment effectiveness and complication rate between the two groups.Results The time for blood ketone to turn negative and urine ketone to turn negative in the treatment group were shorter than those in the control group,and the differences were statistically significant(t=22.866,25.191,P<0.05).The time to reach the blood glucose standard and the insulin dosage of the children in the treatment group were better than those in the control group,and the difference was statistically significant(t=6.302,13.828,P<0.05).The total effective rate of the children in the treatment group was higher than that in the control group,and the difference was statistically significant(χ^(2)=4.114,P<0.05).The incidence of complications in the treatment group was 5.00%lower than that in the control group,20.00%,and the difference was statistically significant(χ^(2)=4.114,P<0.05).Conclusion The effect of continuous subcutaneous injection of insulin pump in the treatment of children with diabetes mellitus and ketoacidosis is better than that of intravenous drip of insulin.
