摘　　要：目的:探究糖尿病视网膜病变(DR)患者血管内皮生长因子与DR分期间的相关性。方法:回顾性收集2018年9月~2019年12月我院住院治疗的DR患者176例,DR分期为Ⅰ期31例,Ⅱ期22例,Ⅲ期36例,Ⅳ期38例,Ⅴ期30例,Ⅵ期19例。DR患者176例作为DR组,另选择同期在院内进行健康体检者100例作为正常组,同期治疗糖尿病无视网膜病变者100例为NDR组,所有入选者均进行血管内皮生长因子(VEGF)及HbA1c检测,采集血浆、玻璃体标本获各个标本VEGF水平,将合并后的DR分期(Ⅰ-Ⅱ期,Ⅲ-Ⅳ期,Ⅴ-Ⅵ期)作为自变量,指标异常优势为因变量,建立Logistic回归方程,分析DR合并分期与检验指标异常。以DR分期为自变量,各指标检测数值为因变量,建立多重线性回归方程,分析DR分期与各指标数值结果的关系。结果:与正常组相比,DR组与NDR组血浆、玻璃体标本VEGF水平及HbA1c较高;与NDR组相比,DR组血浆、玻璃体标本VEGF水平及HbA1c较高。随着DR分期进展,血浆、玻璃体标本VEGF水平与HbA1c随之增高。将正常组VEGF水平及HbA1c作为标准数值,DR患者VEGF及HbA1c检测数值及检验异常者比例均随分期进展趋势而增高。经Logistic回归分析表明,合并后的DR分期对VEGF及 HbA1c有明显影响。多重线性回归结果显示,DR分期对VEGF及HbA1c均存在影响,随DR分期的进展依次增加。在体现DR分期与VEGF关系的4个回归方程中,有统计学意义的偏相关系数为Ⅴ、Ⅵ分期,而在DR分期与血浆、玻璃体VEGF与HbA1c关系的回归方程中,有统计学意义的偏相关系数为分期Ⅱ分期~ Ⅵ分期,提示血浆、玻璃体VEGF与HbA1c均有随DR分期进展逐渐增加的趋势。结论:DRⅢ~Ⅳ期开始,血浆、玻璃体标本VEGF及HbA1c异常的可能性明显增加,并随分期进展随之增高,DR Ⅲ期能作为各指标异常的阈值分期,提示VEGF与DR的分期呈正相关,在DR中有重要作用。Objective To explore the correlation between vascular endothelial growth factor and DR staging.Methods A total of 176 patients in our Hospital from September 2018 to December 2020 were retrospectively collected.There were 31 cases in stage Ⅰ,22 cases in stage Ⅱ,36 cases in stage Ⅲ,38 cases in stage Ⅳ,30 cases in stage V and 19 cases in stage Ⅵ.176 cases of DR patients were selected as DR group,100 cases of healthy physical examination in hospital were selected as normal group,and 100 cases of diabetic patients without omental lesions were selected as NDR group.All the patients were tested for vascular endothelial growth factor (VEGF) and HbA1c.The levels of VEGF in plasma,aqueous humor and vitreous samples were collected to obtain the levels of VEGF in each sample.The combined DR stages (Ⅰ-Ⅱ,Ⅲ-Ⅳ,Ⅴ-Ⅵ) were determined Logistic regression equation was established to analyze the relationship between DR Combined stage and abnormal index.The multiple linear regression equation was established to analyze the relationship between DR staging and the numerical results of each index.Results Compared with the normal group,the levels of VEGF and HbA1c in plasma,aqueous humor and vitreous samples of DR group and NDR group were higher;compared with NDR group,the levels of VEGF and HbA1c in plasma,aqueous humor and vitreous samples in DR group were higher.With the progression of DR staging,the levels of VEGF and HbA1c in plasma,aqueous humor and vitreous samples increased.Taking the level of VEGF and HbA1c in normal group as the standard value,the detection value of VEGF and HbA1c in DR patients and the proportion of patients with abnormal test increased with the development trend of staging.Logistic regression analysis showed that the combined DR staging had a significant effect on the normality of VEGF and HbA1c.Multiple linear regression results showed that DR staging had an effect on VEGF and HbA1c,which increased with the progression of DR staging.In the four regression equations reflecting the relatio
