摘　　要：目的:总结老年社区获得性肺炎(CAP)急性期患者细胞免疫功能及临床意义。方法:86例老年CAP患者作为观察组,同期入院体检者60例作为对照组。分别取患者空腹静脉血,检测CD4^(+)、CD8^(+)T淋巴细胞比例,CD4^(+)/CD8^(+)比值、血清补体C3、补体C4、IgA、IgM、IgG含量。结果:观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均低于对照组,差异有统计学意义。两组患者CD8^(+)亚群比例、血清IgG含量差异无统计学意义。观察组患者血清IgA、IgM含量显著高于对照组。观察组C3、C4含量均低于对照组。观察组PCT含量、hs-CRP含量均高于对照组。结论:老年CAP患者细胞免疫功能下降,血清免疫球蛋白、PCT、hs-CRP水平升高,补体水平下降,免疫功能指标可作为老年CAP的早期诊断、预后评估指标。Objective To observe the status of cellular immune function and its clinical significance in elderly patients with community acquired pneumonia(CAP)in the acute phase.Methods 86 elderly patients with CAP were taken as the observation group,and 60 patients who were admitted to the hospital for health examination during the same period were taken as the control group.The fasting venous blood was taken from the patients to detect the percentage of CD4^(+),CD8^(+)T lymphocytes,and the ratio of CD4^(+)/CD8^(+).Serum complement C3,complement C4,IgA,IgM,IgG content.Results CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the observation group were lower than those in the control group,and the difference was statistically significant.There was no significant difference in the ratio of CD8+ subgroups and serum IgG content between the two groups.Serum IgA and IgM levels in the observation group were significantly higher than those in the control group.The contents of C3 and C4 in the observation group were lower than those in the control group.The contents of PCT and hs-CRP in the observation group were higher than those in the control group.Conclusion The cellular immune function of elderly patients with CAP decreases,the serum immunoglobulin,PCT,hs-CRP levels increase,and the complement level decreases.Immune function indicators can be used as early diagnosis and prognostic indicators for elderly CAP.
