猪肺磷脂注射液联合高频通气模式治疗新生儿呼吸窘迫综合征的应用效果

摘　　要：目的:观察猪肺磷脂注射液联合高频通气模式治疗新生儿呼吸窘迫综合征(NRDS)的应用效果。方法:选择2020年4月~2022年4月在院收治的60例NRDS患儿作为研究对象,采用随机数字表法将患者分为两组,对照组(30例)采用猪肺磷脂注射液联合间歇指令机械通气治疗,研究组(30例)采用猪肺磷脂注射液联合高频震荡通气治疗,比较两组患者的临床疗效、血气分析及呼吸力学指标、临床效果和合并症。结果:治疗后,治疗组与对照组相比,治疗组的总治疗有效率为96.67%。结果:实验组动脉血氧分压(PaO_(2))、血氧饱和(SpO_(2))明显升高,而血管内CO_(2)(PaCO_(2))浓度则明显降低。研究组患者平均气道阻力、呼出潮气量及动态顺应性明显高于对照组。研究组患者氧疗时间、机械通气时间及住院时间低于对照组。两组患者并发症总发生率无明显差异。结论:猪肺磷脂注射液联合高频振荡通气可明显提高NRDS患儿的疗效,并能有改善患儿的血气功能与呼吸功能指标,促进患儿康复,安全性及临床应用价值均较高。Objective To analyze the effect of porcine lung phospholipid injection combined with high frequency ventilation mode in the treatment of neonatal respiratory distress syndrome (NRDS).Methods Choice in April 2020-April 2022 in the hospital treated 62 cases of children with NRDS as the research object.The patients were divided into 2 groups by random number table method.The control group (31 cases) with pig lung phospholipids injection with intermittent instruction mechanical ventilation therapy,group (31 cases) with pig lung phospholipids injection with high frequency oscillation ventilation therapy,compared 2 groups of clinical efficacy,blood gas analysis and respiratory mechanics index,Clinical treatment and complications of 2 groups were recorded.Results After treatment,the total effective rate of the two groups was 96.67%,which was significantly higher than that of the control group (76.67%).Results The arterial oxygen partial pressure and oxygen saturation in the experimental group were higher than those in the normal group,while the arterial CO_(2) concentration was lower.The results showed that the airway resistance,expiratory tidal volume and dynamic compliance of the patients in the experimental group were significantly better than those in the control group.The oxygen therapy time,mechanical ventilation time,hospitalization time and other indicators in the treatment group were significantly reduced.The incidence of postoperative complications between the two groups was not statistically significant.Conclusion Pig lung phospholipid injection combined with high frequency oscillatory ventilation can significantly improve the efficacy of children with NRDS,and can effectively improve the blood gas analysis and respiratory function indicators of children,and promote the rehabilitation of children,with high safety and clinical application value.
