乙酰半胱氨酸联合血浆治疗老年肝衰竭的临床疗效观察

摘　　要：目的:探讨乙酰半胱氨酸联合血浆治疗老年肝衰竭的临床疗效。方法:选取2020年6月~2021年6月本院收治的120例老年肝衰竭患者,随机数字表法分为对照组和观察组各60例,对照组给予乙酰半胱氨酸治疗,观察组在对照组基础上同时进行血浆置换治疗,比较两组肝功能、凝血功能、炎性因子水平及不良反应发生情况。结果:观察组治疗后碱性磷酸酶(ALP)、谷草转氨酶(AST)、间接胆红素(IBIL)等水平均低于对照组;观察组治疗后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、国际标准化比值(INR)明显优于对照组;观察组治疗后降钙素原(PCT)、白细胞计数(WBC)、白细胞介素-6(IL-6)水平均低于对照组。结论:乙酰半胱氨酸联合血浆治疗老年肝衰竭可改善其肝功能及凝血功能,调节炎性因子水平,且安全性好。Objective To investigate the clinical effect of N-acetylcysteine combined with plasma in the treatment of elderly patients with liver failure.Methods 120 elderly patients with liver failure treated in our hospital from June 2020 to June 2021 were randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with acetylcysteine,and the observation group was treated with plasma exchange on the basis of the control group.The liver function,coagulation function,inflammatory factor level and adverse reactions of the two groups were compared.Results After treatment,the levels of alkaline phosphatase (ALP),aspartate aminotransferase (AST),indirect bilirubin (IBIL) and etc in the observation group were lower than those in the control group.The prothrombin time (PT),activated partial thromboplastin time (APTT),thrombin time (TT) and international normalized ratio (INR) in observation group were significantly better than those in the control group.The levels of procalcitonin (PCT),white blood cell count (WBC) and interleukin-6(IL-6) in the observation group were lower than those in the control group.Conclusion N-acetylcysteine combined with plasma can improve liver function and coagulation function,and regulate inflammatory factor levels in patients with liver failure,with good safety.
