鼻内镜下鼻窦开放术对真菌性鼻窦炎临床治疗效果探讨

摘　　要：目的研讨真菌性鼻窦炎患者接受鼻内镜下鼻窦开放术处理的临床作用。方法选取2017年10月—2020年10月期间入住江苏省宿迁市泗阳县泗阳医院的70例真菌性鼻窦炎患者为研究对象,按照随机数表法分为两组,每组35例。对照组患者予以传统Caldwell-Luc手术治疗,观察组患者实施鼻内镜下鼻窦开放术治疗,比较两组的手术总有效率、不同手术阶段的鼻气道阻力(nasal airway resistance,NAR)与嗅觉功能、术后感染发生率以及复发率。结果观察组在手术总有效率为97.14%,高于对照组的77.14%,差异有统计学意义(χ^(2)=4.590,P<0.05)。观察组在术后1、3、6周NAR值与嗅觉功能评分均较对照组低,差异有统计学意义(P<0.05)。两组在术后感染率(2.86%vs 8.57%)与复发率(2.86%vs 5.71%)对比,差异无统计学意义(χ^(2)=0.265、0.000,P>0.05)。结论对真菌性鼻窦炎患者实施鼻内镜下鼻窦开放术处理的效果较好,可明显减轻鼻气道阻力,改善患者的嗅觉功能,并且对减少术后感染以及复发也有一定作用。Objective To study the clinical effect of endoscopic sinus surgery in patients with fungal sinusitis.Meth⁃ods A total of 70 patients with fungal sinusitis who were admitted to Jiangsu Province Suqian Siyang Hospital from October 2017 to October 2020 were selected.Divide into two groups according to the method of random number table,35 cases in each group.The patients in the control group were treated with traditional Caldwell-Luc surgery,and the patients in the observation group were treated with endoscopic sinus surgery.The total effective rate of operation,nasal airway resistance(NAR)and olfactory function at different surgical stages,the incidence of postoperative infection and the recurrence rate were compared between the two groups.Results The total effective rate of the observation group was 97.14%higher than that of the control group by 77.14%,and the difference was statistically significant(χ^(2)=4.590,P<0.05).The NAR value and olfactory function score in the observation group were lower than those in the control group at 1,3 and 6 weeks after operation,and the difference was statistically significant(P<0.05).Comparison of postoperative infection rate(2.86%vs 8.57%)and recurrence rate(2.86%vs 5.71%)between the two groups,and the difference was no statistically significant(χ^(2)=0.265,0.000,P>0.05).Conclusion The treatment effect of endoscopic sinus surgery for fungal sinusitis patients is good,which can significantly reduce nasal airway resistance,improve the patient′s olfactory function,and also have a certain effect on reducing postoperative infection and recurrence.
