摘 要:目的探讨对儿童外伤后瘢痕采用强脉冲光联合二氧化碳点阵激光治疗的临床效果。方法选择2018年7月—2021年6月连云港市妇幼保健院收治的外伤后瘢痕患儿62例。以随机数表法分为窄谱强脉冲光治疗的对照组(n=31)与联合二氧化碳点阵激光治疗的观察组(n=31)。评价两组临床疗效,并对比治疗前后患儿温哥华瘢痕量表(VSS)评分,并以四项目瘙痒量表评分比较治疗前后患儿瘙痒程度。结果观察组总有效率较对照组高(93.55%vs 70.97%),差异有统计学意义(χ^(2)=5.415,P<0.05)。治疗后,观察组VSS量表在色泽、柔软度、血管分布、厚度4个维度的评分分别为(0.57±0.20)分、(0.75±0.28)分、(0.61±0.22)分、(0.65±0.21)分,均低于对照组,差异有统计学意义(t=6.630、5.198、4.640、3.977,P<0.05)。治疗后,观察组四项目瘙痒量表在瘙痒部位、程度、频率以及睡眠障碍4个维度的评分分别为(1.06±0.17)分、(1.22±0.28)分、(1.33±0.28)分、(1.29±0.31)分,均低于对照组,差异有统计学意义(t=3.394、5.815、3.121、5.478,P<0.05)。结论对儿童外伤后瘢痕予强脉冲光联合二氧化碳点阵激光治疗可提高临床疗效,有利于促进瘢痕修复,且可减轻患儿的瘙痒程度,值得推广。Objective To investigate the clinical effect of intense pulsed light combined with carbon dioxide fractional laser therapy for post-traumatic scars in children.Methods A total of 62 children with post-traumatic scars who were admitted to Lianyungang Maternal and Child Health Hospital from July 2018 to June 2021 were selected.According to the random number table method,they were divided into the control group used narrow-spectrum intense pulsed light treatment(n=31)and the observation group used combined carbon dioxide fractional laser treatment(n=31).The clinical efficacy of the two groups was evaluated,and the Vancouver Scar Scale(VSS)scores were compared before and after treatment.The four-item pruritus scale was used to compare the degree of pruritus before and after treatment.Re⁃sults The total effective rate of the observation group was higher than that of the control group(93.55%vs 70.97%),and the difference was statistically significant(χ^(2)=5.415,P<0.05).After treatment,the VSS scale in the observation group was scored in the four dimensions of color,softness,blood vessel distribution,and thickness,respectively(0.57±0.20)points,(0.75±0.28)points,(0.61±0.22)points,(0.65±0.21)points,all lower than the control group,and the difference was statistically significant(t=6.630,5.198,4.640,3.977,P<0.05).After treatment,the four-item pruritus scale in the observation group was scored as(1.06±0.17)points,(1.22±0.28)points,(1.33±0.28)points,and(1.29±0.31)points in the four dimensions of itching location,degree,frequency and sleep disturbance,respectively,were smaller than the control group,and the difference was statistically significant(t=3.394,5.815,3.121,5.478,P<0.05).Conclusion Intense pulsed light combined with carbon dioxide fractional laser therapy for post-traumatic scars in children can improve clinical efficacy,promote scar repair,and reduce itching in children,which is worthy of promotion.
