基于药学信息数据库评价降糖药物的相互作用

摘　　要：目的为更好地指导临床用药,保证临床用药安全合理,规避严重药物相互作用,此研究依托药学信息数据库检索降糖药物的相互作用(DDI).方法在Lexicomp和Micromedex两个数据库中检索《中国2型糖尿病防治指南(2020版)》中所有推荐的除外胰岛素的其他降糖药物的DDI,并进行归纳总结和分析.结果29种降糖药物在Lexicomp提示存在795对DDI,在Micromedex中为620对,其中X级/禁忌的分别为27对和37对,D级/严重的分别为90对和232对.与各类降糖药物的相互作用中,X级/禁忌药物相互作用较少,主要药物包括磺脲类降糖药、瑞格列奈、GLP-1受体激动剂、二甲双胍、沙格列汀.但D级/严重即存在需要改进治疗方案的相互作用的药物种类较多.结论对于多病共存,需要服用多种药物的患者,关注降糖药物的DDI,规避低血糖等致命性风险是十分必要的.Objective In order to better guide clinical drug use,ensure the safety and rationality of clinical drug use,and avoid serious drug interactions,this study relied on the pharmaceutical information database to search for antidiabetic drug interactions(DDI).Methods Two databases,Lexicomp and Micromedex,were searched for the DDIs of all recommended antidiabetic drugs other than insulin in the"Chinese Guidelines for the Prevention and Treatment of Type 2 Diabetes(2020 Edition)"and summarized.Results For 29 hypoglycemic drugs,795 pairs of DDI were indicated in Lexicomp and 620 pairs in Micromedex,including 27 pairs and 37 pairs of grade X/contraindicated,and 90 pairs and 232 pairs of grade D/severe,respectively.Among the interactions with various antidiabetic drugs,grade X/contraindicateddrug interactions are rare,mainly occurring in sulfonylurea antidiabetic drugs,repaglinide,GLP-1 receptor agonists,mefformin,and saxagliptin.tinctures in these drugs.However,grade D/severe,that is,there are more types of drugs with interactions that require improved treatment.Conclusion For patients who coexist with multiple diseases and need to take multiple drugs,it is necessary to pay attention to the DDI of hypoglycemic drugs and avoid fatal risks such as hypoglycemia.
