耳穴埋豆治疗BPPV复位成功后残余头晕疗效观察及对皮肤交感反应的影响

摘　　要：目的观察耳穴埋豆治疗良性阵发性位置性眩晕(BPPV)复位成功后残余头晕患者的临床疗效。方法选取温州市中医院2019年1月至2021年5月收治的BPPV复位成功后残余头晕患者110例为研究对象,采用随机数字表法分为对照组和研究组,每组55例。对照组患者予以药物治疗,研究组患者予以耳穴埋豆治疗。比较两组患者治疗前后眩晕残障程度评定量表(DHI)、活动平衡信心量表中文版(ABC)和汉密尔顿焦虑量表(HAMA)评分,测定患者交感神经皮肤反应(SSR)潜伏期和波幅值变化情况。结果治疗15 d后,研究组各维度DHI评分均显著低于对照组(t=16.13~20.62,均P<0.001),ABC评分[(87.90±6.01)分]显著高于对照组[(80.55±8.73)分](t=3.10,P<0.05),HAMA评分[(7.85±1.06)分]显著低于对照组[(13.30±2.49)分]((t=8.43,P<0.001);研究组SSR起始潜伏期值[(1.95±0.27)ms]显著高于对照组[(1.67±0.21)ms],而SSR波幅值[(1.49±0.15)mV]显著低于对照组[(1.70±0.22)mV](t=4.73、4.04,均P<0.001);研究组患者不良反应发生率[0.0%(0/55)]显著低于对照组[10.9%(6/55)](χ^(2)=4.40,P<0.05)。结论耳穴埋豆可有效缓解BPPV复位成功后残余头晕患者头晕症状,提高患者心理健康水平,改善自主神经功能,值得临床推广。Objective To investigate the efficacy of auricular acupoint bean-embedding therapy in the treatment of residual dizziness after successful reduction of benign paroxysmal positional vertigo(BPPV)and its effect on skin sympathetic response.Methods A total of 110 patients with residual dizziness after successful reduction of BPPV who were admitted to Wenzhou Hospital of Traditional Chinese Medicine from January 2019 to May 2021 were included in this study.They were randomly divided into control and study groups,with 55 patients in each group.Patients in the control group were treated with drugs,and those in the study group were treated with auricular acupoint bean-embedding therapy.Before and after treatment,dizziness handicap inventory(DHI)score,activities-specific balance confidence(ABC)score,and Hamilton Anxiety Scale(HAMA)score were compared between the two groups.The latency and amplitude of sympathetic skin response(SSR)were measured.Results After 15 days of treatment,DHI scores of all dimensions in the study group were significantly lower than those in the control group(t=16.13-20.62,all P<0.05).ABC score in the study group was significantly higher than that in the control group[(87.90±6.01)points vs.(80.55±8.73)points,t=3.10,P<0.05).HAMA score in the study group was significantly lower than that in the control group[(7.85±1.06)points vs.(13.30±2.49)points,t=8.43,P<0.001].The initial latency value of SSR in the study group was significantly higher than that in the control group[(1.95±0.27)ms vs.(1.67±0.21)ms,and the amplitude of SSR in the study group was significantly lower than that in the control group[(1.49±0.15)mV vs.(1.70±0.22)mV,t=4.73,4.04,both P<0.001].The incidence of adverse reactions in the study group was significantly lower than that in the control group[0.0%(0/55)vs.10.9%(6/55),χ^(2)=4.40,P<0.05].Conclusion Auricular acupoint bean-embedding therapy can effectively alleviate the symptoms of dizziness after successful reduction of BPPV,improve patient's psychological status and autonomic
