乌司他丁对非停跳冠状动脉搭桥术后患者心肺功能、血浆炎症因子及心肾功能损伤的影响

摘　　要：目的探究乌司他丁对非停跳冠状动脉搭桥术后患者心肺功能、血浆炎症因子及心肾损伤的影响。方法选取我科2018年8月至2020年10月期间收治的87例非停跳冠状动脉搭桥术后患者为研究对象,按照数字表法随机分为两组。对照组患者43例,术后采用常规治疗,观察组患者44例,术后在常规治疗的基础上加用乌司他丁治疗。对比两组患者术后情况。结果治疗后观察组患者后气道阻力、呼吸指数、肺泡-动脉血氧分压差、肺循环阻力均低于对照组,肺顺应性大于对照组(P<0.05),肿瘤坏死因子、白细胞介素-6、白细胞介素-8水平均低于对照组(P<0.05),肌酸激酶同工酶、血浆钠尿肽、胱抑素-C水平低于对照组(P<0.05),不良反应发生人数显著降低,不良反应发生率低于对照组(P<0.05)。结论乌司他丁可以改善非停跳冠状动脉搭桥术后患者心肺功能和血浆炎症反应,减轻患者术后心肾功能的损伤程度。Objective To explore the effect of ulinastatin on cardiopulmonany function,plasma inflammatory factors and heart and kidney function injury in patients after non-stop coronary artery bypass grafting.Methods A total of 87 patients after non-stopping coronary artery bypass grafting in our department from August 2018 to October 2020 were enrolled in this study and divided into two groups according to random number grouping method.Forty-three patients in the control group received conventional treatment after the operation.Forty-four patients in the observation group were treated with ulinastatin on the basis of conventional treatment after surgery.The postoperative conditions of the two groups were compared.Results After the treatment,the airway resistance,respiratory index,alveolar-arterial partial pressure difference,compliance of lung and pulmonary vascular resistance in the observation group were lower than those in the control group,and pulmonary compliance was higher than that in the control group(P<0.05).The levels of tumor necrosis factor,interleukin-6 and interleukin-8 in the observation group were lower than those in the control group after the treatment(P<0.05),and the levels of creatine kinase isoenzyme,plasma natriuretic peptide and cystatin C in the observation group were lower than those in the control group(P<0.05).After the treatment,the number of adverse reactions in the observation group was significantly decreased,and the incidence of adverse reactions was lower than that in control group(P<0.05).Conclusion Ulinastatin can improve cardiopulmonary function and decrense plasma inflammatory reaction after the operation,and reduce the degree of heart kidney function damage.
