摘　　要：目的了解凉山州艾滋病病毒感染者/病人(HIV/AIDS)接受抗病毒治疗及时性的现状,并分析其相关因素,为提高治疗及时性提供科学依据。方法利用国家艾滋病综合防治信息系统,选取截至2019年底现住址在凉山州,已接受艾滋病抗病毒治疗的HIV/AIDS为研究对象。共有41384例病例纳入本研究。从数据库中采集病例的基本情况、随访与治疗信息,采用单因素和多因素Logistic回归,分析影响HIV/AIDS患者抗病毒治疗及时性的主要因素。结果41384例患者中,男女之比为1.74∶1(26298/15086),小学及以下占87.28%,已婚或同居者居多(73.72%),以异性传播为主(53.83%),开始治疗时平均年龄(35.42±10.47)岁。多因素Logistic回归分析结果显示:开始治疗时年龄<20岁、样本来源为检测咨询、基线CD4+T淋巴细胞计数<200个/μl是及时治疗的有利因素,感染途径为静脉吸毒传播是及时治疗的不利因素。结论抗病毒治疗及时率在年龄、婚姻状况、文化程度、感染途径、样本来源、基线CD4+T淋巴细胞数有统计学差异,在促进HIV/AIDS患者及时治疗时,需根据其所在地域、疾病状况等采取针对性综合干预措施,进一步加强早期检测,使更多患者能够在疾病早期得到确诊并及时纳入治疗。Objective To understand the treatment status of HIV/AIDS patients and its influencing factors in Liangshan,so as to provide scientific basis for improving the timeliness of treatment.Methods Using the National AIDS comprehensive prevention and control information system,the HIV/AIDS patients with the current address in Liangshan Prefecture were selected as the end of 2019.A total of 41384 cases were included in this study.Basic information,follow-up and treatment information of cases were collected from the database.Influencing factors on the timeliness of antiviral treatment in HIV/AIDS patients were analyzed by univariate and multivariate logistic regression models.Results Among 41384 patients,the ratio of male to female was 1.74∶1(26298/15086);87.28%were in primary school and below,73.72%were married or cohabiting,and 53.83%were heterosexual transmission.The average age of patients at the beginning of treatment was(35.42±10.47)years.Multivariate logistic regression analysis showed that the age at the beginning of treatment<20 years,the sample source of testing and consultation,and the baseline CD4+T lymphocyte count<200/μl is a favorable factor for timely treatment;however,the transmission of intravenous drug abuse is an unfavorable factor for timely treatment.Conclusions There are significant differences in the timely treatment rate in age,marital status,education level,route of infection,sample source and baseline CD4+T lymphocyte number.When promoting the timely treatment of HIV/AIDS patients,targeted comprehensive intervention measures should be taken based on their region and disease status to further strengthen early detection,so that more patients can be diagnosed in the early stage of the disease and included in treatment on time.
