人工全膝关节置换治疗夏科氏关节病1例的经验报道

摘　　要：夏科氏关节病是一种继发于神经系统疾患所引起的痛觉缺失并导致的慢性进行性关节病变,是一组以感觉、运动、自主神经受损,关节严重破坏、活动不稳和骨质疏松为特征的临床少见疾病。本文报道1例夏科氏关节病患者因“膝关节疼痛伴活动受限”至广东省中医院行人工膝关节置换术,术后出现“假体周围骨折”及“假体周围感染”等并发症,经假体翻修、延长外固定时间、加强抗感染等治疗后,取得满意效果。本文通过病例回顾,总结夏科氏膝关节手术及术后康复要点,旨在提高临床医师对夏科氏关节病的认识。Charcot arthropathy is a chronic progressive joint disease caused by nociceptive loss secondary to neurological disorders,it is a group of rare clinical diseases characterized by sensory,motor,and autonomic nerve damage,severe joint destruction,instability,and osteoporosis.In this paper,a patient with Charcot joint underwent artificial knee arthroplasty in Guangdong Hospital of Traditional Chinese Medicine due to“knee pain with limited mobility”,postoperative complications such as“periprosthetic fracture”and“periprosthetic infection”occurred.After prosthesis revision,extension of external fixation time,strengthening anti-infection,and other treatments,satisfactory results are obtained.This article summarizes the key points of Charcot joint surgery and postoperative rehabilitation through case review,in order to improve clinicians’understanding of Charcot joint.
