硝普钠与多巴胺联合治疗顽固性心力衰竭的临床效果分析

摘　　要：目的分析硝普钠联合多巴胺治疗顽固性心力衰竭的临床效果。方法74例顽固性心力衰竭患者,按随机数字表法分为观察组与对照组,各37例。对照组给予多巴胺治疗,观察组给予硝普钠联合多巴胺治疗。对比两组患者临床疗效及治疗前后心功能指标[心率(HR)、左室舒张末期内径(LVEDD)以及左室射血分数(LVEF)]、血清肌钙蛋白T(TnT)以及N末端B型利钠肽前体(NT-proBNP)水平。结果观察组治疗总有效率97.30%高于对照组的81.08%,差异有统计学意义(P<0.05)。治疗后,观察组HR(81.62±8.13)次/min、LVEDD(60.05±2.05)mm及LVEF(43.06±6.18)%均优于对照组的(90.59±8.96)次/min、(63.69±3.06)mm、(38.69±6.08)%,差异有统计学意义(P<0.05)。治疗后,观察组TnT(0.96±0.10)μg/L、NT-proBNP(790.62±113.62)ng/L低于对照组的(1.32±0.13)μg/L、(1001.18±216.35)ng/L,差异有统计学意义(P<0.05)。结论对顽固性心力衰竭患者应用硝普钠联合多巴胺治疗的效果确切,有利于改善患者的心功能,值得推广。Objective To analyze the clinical effect of sodium nitroprusside combined with dopamine in the treatment of refractory heart failure.Methods A total of 74 patients with refractory heart failure were divided into observation group and control group according to random numerical table,with 37 cases in each group.The control group was treated with dopamine,and the observation group was treated with sodium nitroprusside combined with dopamine.Both groups were compared in terms of clinical efficacy and cardiac function indexes[heart rate(HR),left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF)],serum troponin T(TnT)and N-terminal pro-B-type natriuretic peptide(NT-proBNP)before and after treatment.Results The total effective rate 97.30%of the observation group was higher than 81.08%of the control group,and the difference was statistically significant(P<0.05).After treatment,the HR(81.62±8.13)times/min,LVEDD(60.05±2.05)mm and LVEF(43.06±6.18)%of the observation group were better than(90.59±8.96)times/min,(63.69±3.06)mm and(38.69±6.08)%of the control group,and the differences were all statistically significant(P<0.05).After treatment,the TnT(0.96±0.10)μg/L and NT-proBNP(790.62±113.62)ng/L in the observation group were lower than(1.32±0.13)μg/L and(1001.18±216.35)ng/L in the control group,and the differences were all statistically significant(P<0.05).Conclusion The combined use of sodium nitroprusside and dopamine has a definite effect in the treatment of refractory heart failure patients,which is beneficial to improve the cardiac function of patients,and is worthy of promotion.
